I remember same situation on Condor extraction zone on Gold against Collectors, PC. I thought everyone was safe despite of Possessed Abomination nearby and possibly two Scions between APCs. From my perspective, there was too much cover to be threatened by them, but yet, in about last 6 seconds someone nuked the Scions taking down Abomination... which killed 3 of teammates. I tried to medi-gel instantly, but wasn't extracted.

My personal failures (all PUGs):
I remember my successful nuking bunch of bosses. The problem was I ignored and passed by two Husk and just took position to aim the center of group being exposed to their fire. Not much of my health left, but Husks don't like being ingored just like that and they revenged on my. Fortunately teammates were around :> .

Finishing Gold Reapers on FB Giant, all teammates down (sneaky Banshees). After some kiting and killing all beside Banshees with my heavy armed SI (Javelin + Reegar, when cooldowns weren't that nerfed and Javelin wasn't such a monster) 2 of them left to pull of the wave. I think to myself "screw Javelin, I'll take them faster with Reegar". I melted first one, second left. Going backwards and melting armor, she jumped behind me. Stopped firing and tried to dodge, but not being a host wasn't such a great deal. Impaled by the last enemy.

I've died many times in every way possible, it no longer affects me save frustration. I did feel embarrassed for this dude one time while running an all Fury run on White/Platnium... he charged the initial spawn in wave 1... and got grabbed by a Banshee

Haha! That was a terrible death indeed. Banshee magnet grabs were still around back then, probably my biggest fail in a game yet! Apparently Goosh managed to beat that, 5 seconds into a wave so at least I am not the worst!

Needs more Tankbee's though...

Heh, you'd have been proud of me last night, was running pugs with armored components...wave 10 on Goddess... I shot around five in a 10 second  interval :cheesy grin:

When teammates kill possessed/non possessed abominations while they grabbed me or im about to stomp their head. Not funny at all. And the joke in that, they dont always come to revive me after that.
When i run to nuke a spawn like a true hero and i get killed in the way there or before i pull the trigger. epic fail.
Dodging right in the banshee hug, or brute claws.
Marauder Shield's elbow of doom, followed by its burst. Im already see myself down when its melee me and i got low shields/hp.
When i charge to a boss like Banshee and before i could dodge backwards they grab me. Of course, before that i did several times on them and i didnt got killed. Thats only my stupidity ^^
Swarmer/Ravager goo...more pain than every other units among reapers for me.
Get downed on wave 1 on silver when i start a match alone. I get some lag for 1-2 seconds when someone enter the game, and sometimes i get killed due to that. Im always ashamed because that poor soul who enter the game see only that im already downed on wave 1 
Didnt noticed that my numpad is disabled and when i tried to press a skill, it didnt worked --> die human

Most embarassing & frustrating way to die I've seen is as follows....

Gold vs Reapers.
Last man standing...get downed by husk melee while being marked by 3 ravagers and some marauders, medgel, grabbed by same husk, get obliterated while being husk grabbed, repeat till no medgels left.

Although most frustrating would have to be the insta-kill by praetorian or scion when dropping into someone's game and knowing there's no hope for that round.

Step1)   Make sure the last enemy husk is alive.
Step2)   Make sure there is no ammo left in your gun. (PS: Just having an ass-fight done and 1HP bar left)
Step3)   Get grabbed from behind.
Step4)   Let your wireless keyboard run out of juice.
Step5)   You f**ked up silver to a Husk execution.

Second most embarrassing way is when a Praetorian sync-kills you the bugged way, and does something not so nice to your back-end.  
Third would be from a Cerberus Engineer.  No, not their turrets, the Engineer himself...

When the hazard maps started first appearing, i activated the reactor then managed to dodge into it a second before the doors shut while trying to get into cover. So yeh, self trolling is probably my most embarrassing death.

Anything that kills you after you've killed it basically (DoT attacks).
