Combat engineer, they have such a slow rate of fire and like to walk towards you... maybe even guardians but hey, guardians are somewhat deadly with their talon.
I get killed more often by combat engineers then I like to admit.
What's wrong with the Prime's combat drone? While not being enough to kill you, it does stagger you to death. That little drone is the bane of my existence, nothing else scares me more then that little guy.

Using the Vorcha Hunter, you turn off Bloodlust, intending to Incinerate, and promptly die because of it.
Probably the most embarassing way to die I could think of would be while playing ME3 MP, controller in hand, with your pants down while you are in the pretzel condition with mayonnaise on your chest in the shape of a heart with a smeared picture of Justin Bieber at the center.
This has never happened to me, but I could imagine it.

no way on number 2. swarmers are OP. I'd say
1) Nemesis
2) Ravager Goo (since it doesn't attack you, yo have to be standing in it)
3) Insta-Bruted
Swarmers embarrass the heck out of me if they kill me. Seeing as I can hold a Reaper army at bay alone, it is crazy to think that one little bug that just sneaks up on me manages to bring me down.
As for getting sync-killed by a Brute, any time this happens (or even if he just sneaks up and hits you) you have to say "Oh Crap! A Brute!", especially if you are playing with Relix. He will love you for it!:innocent:

For #3, perform a combat roll as soon as you use a medi-gel. Your character will quickly roll away from the spot and can get away from that pesky Pyro. This has saved me from being sync-killed.
