The Elf mage is Surana.

As for Leiliana, I think it's established that the City Elf was a kid when Leiliana was a young adult, and the City Elf was probably in his/her late teens or early twenties when Origins started. So, depending on what Adaia meant by "child" when she mentioned him to Leiliana, she could be in her early to late thirties (if the CE was in his/her early teens), or even older (though she does not look like late thirties in-game.)
