I have completed them all, but I wasn't credited (nor awarded the commendation pack) for 7 operations.

Annoying to say the least.

Crusader 6, Eagle 8, Valaint 2, Hurricane 3, Valkyrie 3.

I had a really good start meaning no valiant/hurricane for about 10 weeks, but then my magical eagle/crusader luck broke, and my arsenal was spoiled by useful weapons. Luckily, since the Valkyrie's arrival things start to look bright again, and hopefully my paperweight collection rate will resume to it's former glorious state.

I missed one. Operation Resurgeance because I didn't reach extraction as lagged out a couple of times. That operation made me change Internet service an I've never looked back
