Pretty much exactly the same as any other interior level. If you wanted a bare cave type look you could use the deeproads tileset. If you want something like clean cut walls shored up by timberwork then you are going to have to make some custom models as there is nothing like that in the vanilla content.

Over the past week my feelings toward the cav_* tiles went from hatred, because I couldn't figure out how they fit together, to love, when I figured them out and found that I could put together a cave-tunnel complex really fast, and back to hatred, when I realized how many ugly texture seams and lightmap errors had to be covered up by random floor and wall pieces.
It's still a nice tileset since you can pretty much just slap walls and floors and boulders here and there and it looks pretty good. Other indoor tilesets, like tvi_* for example, require more careful placement of walls and pillars. Then when you mix in some stalactites and columns and light effects you can make some really cool-looking stuff.
