But back on topic:
I doubt that. There are too many contradictions within the series, too many discontinuities storywise to assume that they had everything planned out since the 1st game.

I think that it's also obvious that impossibility of conventional victory was definitely planned from the beginning. ME1 ending set that concept in stone... So, I believe they had thoughts about something like Crucible and Catalyst even during ME1 development.

Or are you planning to erase Drew Karpyshyn from ME so that your theories can fit the game?

Lead writer is not the one who invents the entire story. Lead writer is mostly manager, who coordinates writers team work. Game story concept and final story variant is product of cooperative work of the whole writers team. All Mass Effect trilogy's major turn of events could be ideas of anyone in that writers team... Some writer had an idea, team discussed it, everyone liked it - the idea was implemented.

Crucible, definitely. I'm not surprised, though I did think they could come up with something more interesting than "giant freaking laser of varying colors."
