Because that's exactly what they want you to wonder. They were probably planning to do a reveal either at the end of the ME3 DLC cycle or with the next game. They're simply saving the best for last. In the mean time, they need to keep up the illusion. They're doing something that has never been done before, and they're willing to go lengths to fool everybody into thinking that this is it.
The EC gave them the chance to perfect the illusion of the endings. It wouldn't even surprise me if the teleporting squadmates, destroyed relays and random Normandy escape were intentionally put there to:
1. Hint that something was off about the ending, so people would start digging.
2. Fool the rest of us into thinking they screwed up miserably.
Why do you think we got an RGB ending? It's a hint! It's a freaking Reaper illusion with a few changed variables! It's such a huge joke!
The outrage may have been bigger than they anticipated although given some of the quotes that I posted it seems like they at least had an idea that people were going to be pissed off.
I'm not going to say the extended cut was planned, I don't believe that. But I do think the original plotholes were way too stupid to not be intentional. I mean: dead Garrus on the beam run, alive on the Normandy, really?
The destroyed relays are a major clue, because it is symbolic of rejecting Reaper technology, not wanting to use the crucible, but wanting to blow it up (which is why you shoot the damn tube instead of push a big red button). The Crucible is exactly the same as the relays. Nobody knows who designed it. Of course it's the Reapers. By using it, we develop along the paths they desire. Isn't it obvious?
In any case, the extended cut gave them the chance to take the prank even further, to pretend as if they were going to fix it. All the obvious issues were addressed, and now they can act completely like Shepard is over and done. Shepard is metaphorically dead. They want us to go through what Shepard goes through.
And then we will wake up.
So when the day comes that you will hear Harbinger gloat about how you surrendered your form to him when you dove down the synthesis beam, don't say I didn't warn you.
