It's been a while since I have seen the list on how to make sure cetain squaddies don't survive the SM....I know not to do their LM, but how would I make sure Jacob dies without losing anyone else?.....Just don't do his LM and take him to the platorms fot the baby Reaper battle?

Send him in the vents. Even if loyal he will die. If everyone else is loyal and doing what they are optimal for then you'll keep everyone.

He also dies if he's not loyal and you make him the second squad leader while you're in the biotic bubble.

Miranda can get him killed, use her for the biotic bubble part and bring Jacob. She is apparantly not a very reliable biotic and can't hold the shield up long enough and Jacob will end up getting sucked into the swarm. But im not sure if it only happens to Jacob, it might get your other squadmate.
