Last one left for me. Yes, it's hard.

Lone wolf is like picking up a 100 pound (45 kg) iron weight and putting in onto a table. Most able-bodied men can probably do this. If you can do this, it probably isn't something you would consider that difficult. But, if you're a child, infirm, have a broken arm, or something else that makes lifting 100 pounds a problem, the task becomes next to impossible.
For something like the N7 challenge, think of it like moving 100 gallons (379 liters) of water from one end of a room to another. And you have a cup or a bucket to choose from. So long as you can lift at minimum a cup of water at a time, you can perform this task. But even if you're lifting a bucket at a time, 100 gallons is going to take a lot of work.
Which is the more difficult task between the two? Depends on if you're looking for a higher base threshold to perform the task or if you're talking about which will take more effort assuming both tasks are within your abilities.
After lone wolf, the next two highest difficulty "threshold" challenges are squad elite (must be able to at least extract on silver) and nomad (must be able to complete wave 1 on gold). Squad elite requires gold extractions for the time being until more weekend challenges finish, but will eventually be doable with only silver extracts.

As someone who finds releveling tedious I've got to agree with this. I have 0 desire to ever promote any of my classes. Not that I think there is anything wrong with those who do, I just usually only have an hour or two to play each day and prefer to have my characters ready to go.

Edit: The n7 banner probably would have been a lot worse if I didn't have several million credits stockpiled to blow on recruit packs every time I promoted. As it was, it was kind of enjoyable as I hadn't promoted aside from weekend challenges.

Operator, Lone Wolf, and Nomad are the only ones you can't grind out in bronze. Operator, you no longer need platinum for, and in a few weeks, silver players will have it easily. It also only technically required 34 games (33 with 3, 5, and 10 waves survived, at least one more survival medal) if at least 25 are Gold or, in a few weeks SIlver. Lone Wolf is clearly the only banner that requires ANY skill, but it can be gained in a few hours by a competent player. Nomad takes, at minimum, 10 full platinum games on 10 different maps (100 games total). To get it with the least interaction with higher difficulties, you still need a minimum of 120 gold games (20 silver games on hazard maps).
Basically including time and difficulty, I think Nomad is probably the "hardest" to get. Takes a fair amount of time; even if you average 15 minute Platinum runs and all succeed, that's 25 hours. The only mitigating factor is that you can be carried in every game you play without it making a difference.
For me Nomad feels like it's taking forever. So is N7 mastery, but it's not anything I'm actually working at. Unless you play platinum a good amount or Gold is your standard, Map Mastery is pretty challenging/time consuming. But yes, you could techinically get carried through it.
Especially since I'm not too fond of like half of the DLC maps it definitely feels like it is taking longer. I suppose I should just play more platinum, but it's hard for me to PUG and succeed.
EDIT: to the guy above me, that's the Bioware Employee banner (only employees get it)

I would say the commando mastery was the hardest to get. Too much bronze grinding/leeching. Fell asleep a couple of times.

I can kind of see how it's a grindfest, but it was a pretty enjoyable one. I found it easier than blood pack mastery, for instance, because I was already using most of the required characters. A couple ones I wasn't, like the old Turian soldier and Asari adept, were a pleasant surprise.

This. I'm at 22/25 for the mass effect banner and just over halfway past the N7 banner. Promoting and levelling up gets extremely tiresome, especially in the last two levels where you need 1mil Xperia combined.

Well the two banners for me that made me think "Do I really have to do this?" were promotions and Outsider, I really just don't like to play as Volus or Phoenix classes. Although 250 grabs was just plain tedious.
1,000,000 melee points is not?
I'm glad I got combat mastery before this requirement was introduced, as I have still not reached the million point mark yet.

Yea 1 million melee points was alot but you can easily get that in 4/5 games playing normally whereas grabs are more of a dedicated goal which could take you well over 50+ games unless you went all out on grabs or farmed them. Plus I found certain melee builds like the brawler or valkyrie really fun to play.

In terms of difficulty, time spent, or any combination therof?
Mass Effect "Best of the Best" not included.
Time spent: Promotions.
Difficulty: Lone Wolf. Not super-hard, but not purely grindable either.
Combination: I guess Nomad. You have to play gold and you can't do it in 5-10 games like lone wolf.
Most annoying: Melee from combat mastery and guardians from cerberus mastery. At least promoting is kind of fun. These were a chore.
I loved the melee challenge. My favorite characters are melee spec's. Even the Kroguard, who everyone takes the shields and health for, I spec for pure melee Rage. On my solo's I barely used my weapon. Smash all the faces!
But yeah, if it's not your style, then bleh. Like for me, not looking forward to the Sniper Rifle challenges.

I had all my characters maxed before retaliation was first released and I'm at 54/60. It definitely takes time, but it really isn't that bad IMO. I'm at 21/25 for the ME banner.
EDIt: as for the melee damage, yeah I wasn't too keen on it, it took a while. I think the numbers should be swapped with the grenade numbers. Definitely backwards IMO.
