Is it a giant gap? Playing on "Hard" with my second playthrough is not bad. If I bump it to nightmare, is it a huge jump?
I always felt "Hard" was the best because it is most "neutral" with 100% friendly fire and other modifiers almost nil (I think the enemy has a tiny bit of a bump in resists). Is Nightmare all skewed where you have glaring penalities and and enemy has big bonus modifiers? I don't care for that because it a bit too unreal just like where friendly fire damage is minimized on easier difficulties (if I hit my guy with a fireball, he should feel the full effects!)
What is the difficulty gap between "Hard" and "Nightmare"?

In my experience, the difference between Hard and Nightmare is very slight. My first playthrough was on Hard and my second was on Nightmare; if anything, the second seemed easier (probably because I was more familiar with the game and made better choices on builds and party comp). There is no one thing I would point to as a glaring difference, except that traps do noticeably more damage.

Honestly the difficulty of this game revolves around your understanding of the game mechanics. On my first playthrough on hard I thought the game could be brutally difficult some times. My party had several injuries because I didn't want to reload battles that I barely scraped by and won. I thought the game would be impossible without a healer, let alone with out a mage in the party at all. And yet on my last playthrough on nightmare, using an all melee party, I had an easier time of it than practically any other run while on my current playthrough I beat the high dragon at lvl 10 with Shale, my 2h warrior, Morrigan, and Zevran on one reload (forgot to change my potion tactics from <25 to <75 and I'm playing a no injuries game) with no healer and no cheese.Your understanding of the game mechanics has a bigger effect on difficulty than any slider available.

Totally agree that knowledge about game mechanics is the biggest differentiator. I notice this on my second playthrough on Hard again. So excluding knowledge, it sill sounds like it is not much of a jump then...
