Complete Codex (excluding controls) impossible?

I've tried to get a complete codex for several games now. I've tracked and made notes of all of the entries and their locations, and it seems there are four codex entries that only appear in origin stories.

I'd like someone to confirm or disprove that

(a) you can find codex entries 107 (A story of the Brecilian Forest) and 113 (Arlathan, part 2) nowhere outside the Dalish Elf origin story.
(B) you can find codex entry 114 (The Dalish Elves) nowhere outside the City Elf or Dalish Elf origin stories
© you can find codex entry 081 (Mana and the use of magic) nowhere outside the Mage origin story.

If any of these assumptions are wrong, I'd like to know where they can be found outside of origin stories. So far, it seems when I'll have my Dalish Elf game completed, I'll still be missing the 81...

Regarding the control entries, I'm missing the 264, 266, 275, 285 and 288, and it seems you can get either the 267 or 268 but not both. I guess this is the same for everyone else, but just in case: if not, please tell me!

BTW, before you point me to any lists without checking whether the locations for the abovementioned entries are in them - I know quite a few myself, and those are all less complete than my own regarding the locations.

Apparently so. I personally thought it was impossible to get all the codexes (or even only missing a few). After all, certain DLC codex enries seemed to be incorporated into the main "paging" of the book, so I thought there's bound to be holes. Although maybe the extra control codex entries are for certain difficult to get abilities (like whirlwind, that has an achievement attached, or maybe even the "tainted blood" abilities).

And certain codex entries (like the 81,107,113 you mentionned) are definitly origin specific- check the fifth item from the top at this list:

a situation that only happens in the noble origin.
Good luck. My completionist urges havent pushed to obsessively collect codex entries yet, but I'll need some genuine info as soon as other stuff dries up

I think the missing codex entries are bugs. It may be possible that they're so well hidden nobody has found them yet, but considering the control entries mess, and how all codex entries of the history and magic sections appearing in origin stories other than Dalish Elf or Mage (and all but one of the 20 (!) mage-origin-specific entries) appear exactly once in other places as well, I think that's unlikely. One of those obsessive completionists like me would have found them.

Note how even the human noble never gets the "Retrieve your equipment" entry, which seems to be made for this origin story.

Considering the missing control entries, there are situations where I would expect to get them, and - in hindsight, after having looked them up in the wiki, don't understand why you don't get them there:
264 Retrieve your equipment: after opening the cell door in Fort Drakon, with a small text change.
266 Codex: when you get your first codex entry, i.e. at the start of the game.
267 Combat Basics: when you first enter combat with a non-mage in your group, instead of when you first enter combat with a non-mage main character.
268 Combat Basics - Mage: when you first enter combat with a mage in your group, instead of when you first enter combat with a mage main character.
275 Recipes: not surprising you can't get it. You're supposed to get this when acquiring your first recipe, but everyone starts the game with one. So you either have it at the start of the game, or never.
285 Level up: you should get this when reaching a new level for the first time.
288 Party Members: You should get this when acquiring your first named party member (you get 273 when getting the first of any kind, the text of 288 implies it's for those who are "more than servants)

BTW, you get 271 when a party member has accumulated 5 injuries. This is the only "hard-but-not-impossible-to-get" entry in the controls section.
