When I first did the Sacred Ashes quest, after leaving the gauntlet I didn't see the short-cut back to near the the temple entrance and 'walked' all the way back through the tunnels. To makes matters worse I didn't kill the dragon as I thought it may be too strong for my party at that time so later on I went back to the temple and again 'walked' through all the tunnels to get to the dragon. It was only after killing the dragon that I finally noticed  the shortcut - DOH !!!

I missed the shortcut at the Ruined Temple my first time through, too. I only recently got the game for PC, so I started off on my PS3 --  (I'm on my first playthrough on the PC right now and it's a totally different experience, it really is, in a good way, of course). I missed SO MANY FREAKING things my first playthrough, I don't know how I managed to get to the epilogue. First, I didn't grab Lelianna or Sten in Lothering -- nor did I really do anything there at all, because I followed the plot arrow that was guiding me to the Imperial Highway that takes you to camp. Since I played as a mage, it sucked that I didn't have a Rogue until I came across Zevran, and I was a doofus and didn't realize that I had to level up his deft-hands ability to loot and disarm traps.

I didn't even know there was a shortcut until I returned to kill the High Dragon on my second playthrough.

Oh, and the entrance to the Deep Roads. It was right there, and I ran around Orzammar on my first playthrough. I felt like a complete idiot right after I found it.

My first playthrough I was stuck in the Market District in Denerim for about 2 hours, took me that long to figure out how to use the map...Oh the embarassment.

It was the same way with me for the Camp, I couldn't figure "where" the camp was or how to use it...

Morrowind had a Camp button...you could camp almost anywhere...of course some places would get you thrown out on your rear.

I missed the caged guy in Ostagar, didn't get back to the kennel guy with the flowers in time and essentially killed Dog (*sniff), missed most of the quests in Lothering, although I did get Sten and Lelilana, among many things. Actually, after I got close to halfway through, I ditched that playthrough and started that same character over again, on nightmare instead of hard since hard seemed to be getting too easy for me. I'm still finding things I missed in previous playthroughs though.

I was almost through the fade portion of the Mage's Tower before I started to actually use my map to see where the mouse holes led. Almost all of them go in a straight line across the map. >:(

It wasn't until halfway thorugh my very first playthrough that I noticed that BACKstabbing actually does make a difference when playing a rogue... Oh, and I had trouble finding the camp aswell, at first.

During my first playthrough, my Dwarven Commoner was astonished when he returned to Orzamaar, was wandering around the Royal Palace after talking to Rica, and stumbled across some thieves tunneling in!

It wasn't until my third playthrough that I found out you can get a sword off of that Elven Messenger.

I was stuck in Denerim for about half an hour as well, I ended up just travelling to the different areas hoping that it would somehow get me out. And I didn't even know Leliana existed untill I ws about 20 hours into my first playthrough and read about her on the wikia site.

On my first play-through, I didn't use my bow for 8 levels, because I could never find/buy regular arrows. I figured it out purely by accident...

I missed loads of stuff and then as I was playing the wardens keep dlc before the final assault I discovered the tab key and just facepalmed. Going back through the circle tower etc. took ages but I found so much stuff that I just didnt see before.

It also took me half the game to discover how to give gifts...

I didn't realize the "no friendly fire" aspect of the lower difficulties didn't apply to allies until I killed more men at Redcliffe than the damned zombies did.

Seriously, they need to fix that (along with the "secondary spell effects still work on party members" part)...

I swear to god I did this exact same thing. I couldn't figure out how to get out of the damned city LOL

I also went nuts trying to buy regular arrows. I didn't equip my bow because I didn't have ammo. *blush*

I recognise a few of those already listed. I too got stuck in Denerim, killed more of the Redcliffe Militia than the undead managed to and played through most of the game before discovering the tab key (which has since become my favourite key ever. I press it even in games where it does nothing just out of sheer habbit) and

I discovered, half way through the Deep Roads, that those little red icons had something to do with my tendency to die very easily and that doing so caused more little red icons to appear...

fought through the first part of Castle Cousland naked as I was too used to WoW's not being able to equip armor in combat

and once I actually believed it was possible to be cruel to Dagna. but damn it if even my most evil of evil characters can resist her sheer force of persistent optimistic cheer

The quest in the Anvil of the Void after you defeat Caridin/Branka about the list of dwarves who became golems. Didn't even notice it until my second playthrough.

'Took me an embarassingly long time to figure out how to get out of Denerim' crew checking in.

I never would have known about the prisoner in Ostagar if I hadn't see him mentioned on the forums.

In one of the northern rooms of the proving preparations area there's a master-at-arms (though that's not his exact title) who will set you up with some illegial matches in the arena against lesser Dwarven noble houses who employ blood mages, Qunari mercs, etc. Useful, especially when siding with Bhelen and not even trying go to the arena to fight until AFTER you made him king *facepalm*

Yeah, that elven messenger running around Ostagar.  Lie to him and tell him you have a message, he'll think you've come for Ser so-and-so's sword.  It's not a GREAT sword, but it's better than what you can afford otherwise at that point.

If your persuade is high enough, you can convince the guard outside Loghain's tent to let you talk to Loghain BEFORE the strategy meeting with Cailan,  Loghain doesn't say much of relevance, but he is actually polite-neutral-friendly and his responses are keyed to your gender, and maybe origin too, not sure about that part.

For some reason, I remember seeing a video of that prisoner thing in Ostagar nearly 2 years ago. No idea why now. But I do remember it. Some kind of sneak peek?

I did so many things wrong that my first character got trashed at the final battle. I found so many things on subsequent playthroughs I know I missed half the stuff the first time. I'm still finding new things on playthrough 6 and 7 and 8! Still hearing new dialogs. And Iv'e never played an evil or selfish person yet.

lol my list would be so long it's not funny.

1) I just learned what the tactics button did, although I still don't know how to use it

2) I too had no clue about arrows

3) took me about 3 days of playing before figuring out how to camp

4) I was about 3/4 through my first run before realizing what gifts were for

5) I could never find the notes I picked up, took me about 1/2 the game before I discovered the codex section

Lots of these for me, but here's just one:

I found the prisoner at Ostagar, I bribed (on one occasion) and persuaded (on another) the guard to get food for him. But I didn't know I had to go back and give the damn food to him myself until the third time through.
