Lots of these for me, but here's just one:

I found the prisoner at Ostagar, I bribed (on one occasion) and persuaded (on another) the guard to get food for him. But I didn't know I had to go back and give the damn food to him myself until the third time through.

I didn't realize until the second playthrough that you could get the key to the chest from him.   I thought getting him food was just an RP/flavor thing.  I didn't even figure it out myself, just happened to be browsing the Dragon Age wiki and found it.  Luckily I was only a little past that part and could reload.

I did the "where are the normal arrows?" thing too.

I think my biggest omission my first time, though, was not realizing that you can ASK FOR A REWARD from most of the people you do favors for, and that the rewards are pretty substantial and have no negative repercussions.  Getting 40g from one guy in Arl Howe's dungeon is the obvious example.

My first playthrough, after killing the archdemon and listening through the whole royal ceremony, Anora told me there was a crowd waiting for me outside and not to keep them waiting. I went straight to the doors thinking there was another video scene to follow. I didn't realize that I was actually passing by the companions I thought I was about to see outside. /facepalm It wasn't until later I saw someone mention on a thread about talking with companions in the room afterwards that I reloaded my last save, killed the archdemon again and made sure to stay in the room this time, heh. Finally got my closure!

After my first play through, I found out that I had "only" finished 75% of the side quests. So, next time thru going for 100% before the epilogue.

I didn't figure out for a long time that you can run around the outside of the crumbled building by the Redcliffe tavern to get to some chests. There's really nothing spectacular in them, but still....

Me. The prime example of thoroughness. The player who tries to cover every inch of any area in any game. Shamefull, really.

In one of the northern rooms of the
proving preparations area there's a master-at-arms (though that's not
his exact title) who will set you up with some illegial matches in the
arena against lesser Dwarven noble houses who employ blood mages,
Qunari mercs, etc. Useful, especially when siding with Bhelen and not
even trying go to the arena to fight until AFTER you made him king
*facepalm*

What?! Seriously! Oh my God I had no idea. Well that eliminates any reason to side with Harrowmont again!

I thought that you needed special arrows to use the bows! So I respecced Leliana as dual-wielding (d'oh!). My first run through I missed out on loads of approval with my party members, so everyone was pretty indifferent to me by the end. I also missed all the good gear first time round, like Juggernaught and Legion of the Dead armour. Needless to say, my part members equipment sort of sucked.

From the amount of threads about it, this one is easy to miss but its funny all the same -

The first time I did the Elven ruins I obviously hadnt discovered the use for the Tab key. I missed the quest to unlock the Arcane Warrior and instead kept clicking the Broken Altar hoping the jug of water would sit nicely on top.

And another on the turn topic on head theme, I have never gotten the 12th Love Letter.

Now on topic, during my first playthrough it never occurred to me that the Blood Armour was more than a chest-piece and it took me into my second playthrough to notice the rest of the set in Bodhan's store. So yes I was very literally asking myself "How could you miss that? and even loaded an old 1st game save because I was convinced the game must be bugged and the other items were not there. Needless to say they were.

I have played through the game a few times and started loads of characters. I know about the quest for Dog, but I still forget to turn in the Quest to get Dog about half the times. I get sooo embarrassed every time...

I didn't realize the "no friendly fire" aspect of the lower difficulties didn't apply to allies until I killed more men at Redcliffe than the damned zombies did.

Seriously, they need to fix that (along with the "secondary spell effects still work on party members" part)...

Oh dear - currently on my 5th playthrough and i hadn't actually worked that out yet. Though it does explain why no one surivived when i was playing with a mage.

My first few times i completely didn't realise what you had to do with the watchguard of reaching and summoning sciences in the mages tower at all - so i ended up completely ignoring them. Likewise fighting the dragon in the orzamar throne room and getting the sword (is it Ageless you get there?) - didn't even know that was a quest till i saw it on the wiki :P

Ummm...what? ::goes off to search on wiki::

EDIT: Okay, can I just say that I LOVE the fact that I can find out about stuff like this after 8+ playthroughs? That's what I get for not exploring every inch of the world...

Ah, so there is a quest to be had with the throne room. I was always wondering about that, because my PC did get the quest-note, but nothing else ever happened. I guess I better wander over to the wiki myself and see how she (my PC, that is) can trigger the whole of the quest.

QFT! I found it out at same place too! All the NPC's were dying like flies even though I was annihilating all the zombies before they had a chance to damage them! That's one of the worst problems with mages. Their AoE is useless in any area where there's friendly NPC's that might get hit. >.<

I found that out a couple of playthroughs ago. The first time I talked to the messenger, I didn't try to get the sword. It was wrong to lie, lol. The second time I talked to him, with a new character, I decided what the hell. The description of it is hilarious.

"Somewhere a poor knight is fighting with his dinner fork."

I felt rather evil.  But it's not a bad sword for the level!

I missed Dog on the first playthrough when I did the Wilds in one shot and couldn't speak to the kennel master when the map changed on the return. :(

During my first playthrough, my Dwarven Commoner was astonished when he returned to Orzamaar, was wandering around the Royal Palace after talking to Rica, and stumbled across some thieves tunneling in!

What?? Well damn...  lol

And lol about the arrows, I can totally see how you could do that!

Yeah, that elven messenger running around Ostagar.  Lie to him and tell him you have a message, he'll think you've come for Ser so-and-so's sword.  It's not a GREAT sword, but it's better than what you can afford otherwise at that point.

If your persuade is high enough, you can convince the guard outside Loghain's tent to let you talk to Loghain BEFORE the strategy meeting with Cailan,  Loghain doesn't say much of relevance, but he is actually polite-neutral-friendly and his responses are keyed to your gender, and maybe origin too, not sure about that part.

Loghain told my city elf that she was pretty! Image IPB

Back on topic, on my first playthrough I got the key from the prisoner at Ostagar but then I never used it to get the stuff in the chest. I guess it just never came to mind. Finally after 10 hours of game play I think, "Where's that chest that the prisoner was talking about? Crap, Ostagar." Generally I'm pretty good at remembering that kind of stuff but I guess that day, I was just having a brain fart. Image IPB

On my first playthrough, I wandered through Orzammar for a couple hours until I caved and asked on the boards how the hell to get out....I forgot how I had come into Orzammar (through the Hall of Heroes) *sighs*
