It took me three playthroughs (and some comments on YouTube) to make me realize that in Redcliffe, with persuasion you can convince that fat barkeep to join in the town's defense, and if you talk to Bella and ask about the elf in the tavern, and then go talk to him, you can persuade/intimidate him into revealing he's a spy for Loghain and then intimidate him into joining the town's defense. This was also when I figured out the abandoned general store has oil the knights can use as part of the defense against the undead (though it's inadvisable to let your party members get too close, as even on Easy they WILL get burned by it).
Took me a while to figure out that you can do the Urn BEFORE Redcliffe.
Others hve mentioned letting your party members bust you out of Fort Drakon. If Leilana and Morrigan do it and go in as Chantry priests, at one point if you let Leilana bluff....
Leilana: Let us pray, sister.
*they kneel*
Morrigan: (horribly overacting) Maker! Forgive these wretched souls....they know not what they do.....spare them your wrath....and do not inflict boils and lesions upon them.....
HAHAHAHAHAHAHA!!!!!
Or if you go in with Wynne and Morrigan as the priests and they get to the priestess who is already there, with the right dialogue choices the old lady starts to go off on one of the higher ups in the Chantry hierarchy. She's really going at it, pacing back and forth, talking about giving her a piece of her mind etc. Morrigan and Wynne exchange a WTF? look....which may be just about the only time those two agreed on anything, ever.

I missed the wounded elf Deygan in the Brecilian forest on my first 3 playthoughs. Not a huge part but I'm still sad I missed him. 
Then again, I love discovering things I missed previously, its like playing a whole new game!

The secondary exit near the high dragon. On a same note is there away to go from Orzammar to the world map instead of having to cross the frostback mountain area first. It gets increasingly annoying the more playthoughs i do xD.

You can exit to the world map through the Deep Roads.
