Looks like the choice is:
Sacrifice yourself and an Orlesian Grey Warden will take your place.
Don't sacrifice yourself and continue your character.
That's the official word at any place. Anyway looks like a lot of this stuff takes place in the fade.

We have no way of knowing how many possible scenarios there are, just that this is what they chose to make a trailer of. There isn't enough info to draw further conclusions. This follows logically from my ending, but that doesn't mean that every possibility won't be represented.
