All I do is run the batch file, hit Y to rename MassEffect3.exe and texmod.exe, open up the Origin.exe and click ME3 to play. Instead of launching the game, texmod will open up. The top of texmod will have a folder icon, click it, and rename the MassEffect3.exe (that's texmod) back to texmod, and MassEffect3_TMP.exe to the original MassEffect3.exe. Once you've done that you select the textures mods you want to use. Hit run and the textures will overwrite the default ones.
You have to do this everytime you want to play the game with the texture mods.
*EDIT: Since you are using the Mother of All mods coalsced file, then you should check the ID of the ajax mesh and make sure that it's not sharing the same ID number as any of the other torso items.
