For me, there are so many but the 2 that I can think of are when playing against geth, I get VERY paranoid. These have happened multiple times.

I shoot at and try to dodge an escort drone thinking it is s prime's drone.

While playing as I TGI, I mistake my character's cloaked body for a hunter and attempt to run away from myself while cloaked

Couldn't kill Nemesis with 8 Raider clips. Got pssed off. Pulled out Missile Launcher. Missed. Unloaded All Rockets on her corpse. Left. Started weeping into pillow. 

I believe it was not really lucky day.

This happens to me sometimes.

I remember my first game playing the N7 Fury. I was totally inexperienced in using her, and I died so many times that game, sometimes in a truly ridiculous manner.

After the game I left the lobby in shame.

yesterday on a cerberus match, trying to deactivate a device. No one else is bothering to do it. I try and do it even though there are loads of tropps nearby. I get taken down by a turret and 2 phantoms several times in a row. I fire 3 missiles to rage nuke the 3 of them

Taking out a Raider on Quarian Marksman specced for accuracy. Drew Rio Cerberus Gold in the quick match. Could not hit anything.

The crosshairs were so large I couldn't aim anywhere. MIssed all shots that weren't on atlases or extreme close range. Had to stop using marksman, and spam tactical scan.

Died so often because I couldn't kill anything and all enemies had a massive range advantage. Finished with about 60K in a three player match.

Worst game I had since being a bronze player. I was carried.

I am totally owning and destroying the scoreboard, clutching every wave and earning medal after medal. Enemies basically fall dead before me though teammates are nowhere in sight. Then after extraction, the tally sheet comes up and I realize this was a Bronze Geth solo and I'd just burned through 5 missiles, 6 medigels and 5 Ops packs.

I have another shameful confession to make.

Last week, in a Gold game vs Reapers on Hydra, I was happily killing stuff with my Claymore-wielding Krogan Sentinel.

All goes well. I run into a lone Marauder, one of the last enemies left that wave.
I take a Claymore shot. Miss.
Take a second shot. Miss. I was like "WTF?" at this point.
Take a third shot. MISS.

I was so mad then that I threw five Lift grenades to kill the damn thing. The other players were laughing maniacally. I felt so ashamed.

Having a good game and coming in first place but missing out on extraction because I lost track of time.

The first time I tried to play the Slayer, it was on Silver. I failed SO hard. I kept dying, I barely killed a damned thing, and since we'd started with two slots empty, the other guy basically was soloing with a really ****ty decoy for 3 waves. No fourth ever joined, so I was the worst kind of dead weight.

By extraction, I had around 20k points, and it was U/U/S. It was the worst match in memory by a WIDE margin.

It was until after the first DLC that I realized how to actually use a Missile Launcher. By then I've already cleared Gold many times. True story.

My apologies to anyone who played Gold (most importantly Fortack, Stardusk, and Kronner) with me before that...

Playing QM and scored dead. I swear to god it was because everyone was immediately targeting me for no good reason. Any time I peeked my head out of cover no less than 17 different Geth Prime shots came at me.
