Meh all the NPCs stay alive with me (even Anders most of the time) but I don't really want the same ones to come back. I'd rather have new NPCs to go with a new PC.

How can Sten die?

If instead of going straight for the Arch Demon you indulge in side quests and the like, Sten will eventually grow impatient and challange you for control of the party. He can be killed if you let that lead to a fight, but if his approval of you is high enough you can convince him to fall back in line.

Actually, all he's ever done is fight me, and he only does this confrontation in Haven if his approval isn't very high since he feels going on the sidetrack to get the ashes is a waste of time when you should be going after the Archdemon.  So this could be the very first thing you do outside Lothering (you just have to talk to Ser Donal to trigger the ability to do the quest without doing Redcliffe first), and he will still turn on you, regardless of how many side quests you've already done. If it leads to a fight, you still don't kill him, he is beaten down till he gives in.  And you can decide to keep him or tell him 'take a hike' and boot him from the group forever.

To kill Sten, just never recruit him.  He'll stay in that cage to rot till the darkspawn find him.

Ultimately the only way a companion will return as a companion would be if they can never die no matter what decisions you made. There aren't that many and it has been stated who can't die many times. The list for all companions (minus Awakening) are:

DA:O
Alistair: Can die during the summit
Morrigan: Is hinted at dying in Witch hunt but has a chance of surviving
Leliana: Survives even if you kill her
Sten: Can die if you leave him at Lothering or when he confronts you for leadership of the party
Zevran: Can die as soon as you meet him or in Denerim when you meet another Antivan Crow
Wynne: Can die if you side with the templares in the circle quest and is presumed to have died in Asunder
Oghren: Can't die in origins not sure about Awakening

DA2
Varric: Can't die
Isabela: Can't die even if you hand her to the Qunari
Anders: Can die after he blows up the chantry
Fenris: Can die if you side with the mages
Bethany/Carver: Can die in deep roads
Aveline: Can't die
Sebastion: Can't die (unless i'm mistaken if you let Anders live)
Merril: Can die if you side with the Templars

So the ones who have a 100% survival rate are: Varric, Isabela, Leliana, Aveline (possibly Morrigan)
Hawke can't die but s/he isn't a companion but still is 100% so hawke has a chance of appearing

Assuming that Bioware only let these ones return then only 4 can even make an appearance but as the case with Alistair and Zevran those who can die are open to cameos. The chances of them having a major role are slim if they can die whereas the fact that some can never die hints that a major role will occur. The most a potential dead character can have I assume will be a quest like Zevran and Alistair had, if they are dead then the quest doesn't activate.

In any case a cameo just so we know how they are and what they are doing will suffice just so long as it isn't a 1min conversation I would like a quest where we help them or they help us. As for the LI then they could appear with Hawke or appear without saying that they are tending to business elsewhere but we could at least know they are okay providing closure for those who want to know what happened to Hawke and his/her LI.

As for who I would like to see return Isabela, Varric, Alistair, Morrigan and Leliana are the ones I want to see most and most of them are alive

I'm happy with cameos. I don't really care if it makes the world small, I just want to have a little fun and be entertained.

If we were to get any old companion the only 2 I have interest in seeing again are Sebastian and Nathaniel. Besides who I chose as LI's, the way those 2 were just stuck with me for some reason. Loved their character.

The sibling from DA2 could work too and I'd be down with that as well.

And THAT is the key, my friend: Everybody can "get better" and return, if the writers think it necessary.

I'd like to meet Finn from "Witch Hunt" again – the only cowardly-neurotic companion we ever had.

And I'd like to see Alec the Shepard from Awakening – who, depending on the player's decision, has been promised a bright and heroic future...

Gaider said they're done with bringing people back and I believe him unless he actually lied straight to my face.

Wynne officially "dies" in Asunder.

Morrigan cannot be killed in any Origins playthrough.

99.99999% certain that you can kill her at the end of witch hunt.

I don't know... something about that scene tells me she didn't die. Mortally wounded to the brink of death? Sure, but not dead.

If BioWare did pull of (what I hope is going to be) a Jon Snow, then I hope they make the choice 'matter' (note, I always think the choice matters, even if there are no consequences. I'm just thinking about the others here) by dramatically changing Morrigans arch. So basically we have altered Morrigans. The one who wasn't stabbed and is slightly friendly. And the one who was and has a profound sense of mistrust in the world.
