One concern that's been expressed is that Bioware lacks the resources to show the consequences of all our decision, and that this means many don't get addressed, or only in a token manner. But what if the ongoing effects of choices in earlier games (in this case, DAO, DA:A, and DA2 would probably be relevant) were explained through codex entries? DA2 had something like this in the Study of the Fifth Blight, but it didn't add any new information about what had happened afterwards as a result of our decisions.

Spoilers follow:

Did the Warden side with the Dalish or the Werewolves? Did the Warden side with Bhelen or Harrowmont? Did the Warden-Commander form an alliance with the Architect in order to defeat the Mother or did he/she just kill both of them? Did Hawke side with the mages or templars? Did Anders live or die?

It would be cool to have Codices covering smaller choices too. In any event, Codices talking about long-term effects of decisions in the same way that the epilogue captions in DA:O did would be stellar.

This won't happen, but a slide show of what has happened with your imported save would be effective as well. You get to see your Warden/Hawk along with panels showing the story arc of the last two games touching on significant events.

Well, I once made a thread about a Journal/Diary for our PC tht would map all our choices - much like what you are asking for. Be it a codex entry, or an ingame recognition in a form of, say, a buyable book by the esteemed Genitivi, recollecting the heroic deeds of our PC ( or Hawke or Warden...) would show that our actions are indeed affecting the whole world and our character doesn't live in a vacuum...

Codex I take more as fact though (regardless of devs saying to take it as hearsay). I'd rather that not happen but the journal idea sounds good.

Could be nice. Though I'd guess that sort of thing would end up being treated like epilogue slides and retconned pretty quickly if it was inconvenient.

On a related note, how about having the codex remember of PC's name? It annoyed me that the DA2 one didn't

How are Codex entries NOT token references?

I'd say that's less follow up than a random conversation or cameo.
