One of mine are stupidly putting myself in danger to rez teammates I should probably let gel or bleed out. Maybe it's just wanting to be helpful, but a lot of the time I'm pissed off after asking myself why I bothered.

Shooting teammates by accident (Vanguard charges or someone that does a shadow strike from behind and I hit them).
Not sure, but I keep hearing my name called out. I mean no one else gets called out, so I'm probably just a below average player. I can complete the objectives fine, but my team skills are lacking.
Another one is running in front of someone's line of fire.

After using the Javelin for so long and moving back to the BW, I keep trying to reload after one shot. It of course does not help when I use it on the Destroyer and get 4 shots which throws me off as it normally has 3.
