NPC ignorance AKA Spiders are pets in Fereldan...who knew?

By the same token, how come we had a tranquil notice that you're picking a treasure chest in Ostagar and yet you could open Arl's valuts and all right in front of guards and no one seems to care?
Planescape Tornemt had a good system where NPCs reacted appropriately to such behaviors. Why not in DA:O? Is the extra programming too complicated?

Yes! Not only more complicated (as this game is orders of magnitude more complicated than the old Infinity Engine games) but far more time consuming as for each such reaction (Denerim City Guards attacking Morrigan in Spider Form, Wynne crapping her pants when she sees you use Blood Magic) requires animation, sound effects, situational programming, voice acting and potentially other resources as well. And inevitably they'd still leave something out and people would still say "how come X reacted to B but Y didn't react at all to Z?"
