The one mission I really hate, but keep coming back for more........

This time around I did all the loyalty missions, everyone was loyal, and poor Jack dies.......WTF???

Just as she and the others are rushing through the door to where we are after the long walk with Samara Jack gets shot and dies.....WTF?

What did I do wrong? Everyone else lives.

"Just as she and the others" makes it sound like she was leading your second team, and that figures because the only person who should die at that point of the story is whoever is leading the second team. That person always gets shot in the guts and dies after some terrific last words unless they're (a) loyal Garrus (B) loyal Jacob or © Miranda, whose plot armour means she lives at this point whether she's loyal or not.

If Jack wasn't your second team leader then we need more details, starting with who is. Probably easiest if you say everyone doing what job at each stage in the suicide mission for fullest advice. But if she was your team leader then just swap Jack for any of the three I mentioned and you should be alright.

I just checked and I made a mistake........ I had Miranda and Jack with me at the start and Legion in the vent.

OK for the Vent I had Legion...... And travelling with me was Miranda and Jack
I then chose Samara fo the shield and Mordin escorted the survivors that we rescued
Jack lead the other team and when we let them in the door that's locked  she got a bullet and said some terrific words and to quote "How did you talk me into this?"

Second play through I managed to keep everyone alive when I replayed this and swapped people around.

Yeah, I almost wish it was more random. Meaning, I don't like how only certain characters can be assigned to certain things.
Vent assignment: only Kasumi, Legion, or Tali can do it. Lame. Only three choice and a host of people to choose from.
2nd team leader: Garrus, Jacob, or Miranda. Again, lame. Only three can lead?
The biotic thing I get, you only have so many biotics. But in that case you only have two to choose from Jack or Samara/Morinth.
This leaves Thane, Grunt, Mordin with nothing to do but either escort Dr. Chakwa & Co. back or hold the line. Dumb. It would be cool to have Grunt lead and have Mordin sneak through the vents without people dropping.

Yeah it's not very random at all....... And the whole loyalty thing doean't quite add up if you make choices outside of those parameters.......

I think the loyalty quests should be more difficult. More choices should have to be made in order to keep loyalty. Like Jacob, there isn't a single choice you have to make other than follow and shoot and confront his father. Don't even get me started on Thane's. Whereas with Tali if you so choose to show evidence that her father was a war criminal you lose loyalty. Structurally I think Tali's is the ideal, too bad most of the other loyalty quests fall way short.

With Thane's loyalty mission you do have the choice to say screw Thane and watch that Turian throwing shapes across the dance floor instead. I know it's not much, but....

Aside from the lack of choice in Thane's quest, it is utterly boring and generic. Lots of paragon/renegade options, but sheesh, a little effort here, Bioware.
