Might not be the right forum for this but at rank X the Colossus and the Predator S/M/L armour have the best stats bar none The choice between them really depends on whether you prefer points in shields or damage protection, or if you don't mind then on how they look. Don't think Predator comes in a Quarian fit so it's got to be Colossus for Tali, and IMO it looks good on her even though I usually put Predator on everyone else.
Mods for armour: everyone who is not a soldier class and not Wrex should have a medical exo mod on the armour. If you want to make Wrex unstoppable by absolutely everything short of a nuke (or a single shot from Ashley or Shepard when demanded by the plot ;-) ) then give him med mod as well, and two if you're feeling really good. Then you can have non IC fun deliberately sending him into fights to see how hard it is to kill him. For second mod slot it depends on class/character but I tend to go with things that boosts shields or talents as appropriate.
Mods for weapons: incendiary/inferno is probably my favourite ammo mod and tends to be the default ammo in my team's regular weapons, which is funny because I'm a huge fan of cryo in the second game. I also keep some Shredder VII and Tungsten VII around because they don't appear after VII and they're better than their VIII+ replacements for their specialised tasks. I also like to have High Explosive ammo handy for shotguns and sniper rifles. On the guns I go with things that improve accuracy at lower levels and things that improve rate of fire/cooling at higher levels, usually in combo with Combat Optics on at least one person's gun to counter geth jamming. For strong enemies encountered at range I like hi-ex firing sniper rifles with two scram rail mods - stealthy can go whistle when you've got a one shot kill for almost eveything on two legs, even though you have to wait a few seconds before you can use it again because of the huge heat generated. I also get some sick entertainment from firing it into the ground at enemies' feet and seeing how high I can make them fly.
Upgrades: guessing here you mean like what's the best bio-amp, omnitool, AR etc? If so simple answer, Serrice tech for amps and omnitools has the best stats. For all the guns I think the Spectre equipment is _the_ top of the line. Unfortunately price and availability applies to both.

You really can't go wrong with colossus of the appropriate weight and 2xmedical exoskeletons. If you start playing on the higher difficulties, you'll quickly figure out that DR trumps shields every time.
The medical exo's are great, not only for the healing factor (which is very effective with high dr), but also because of the power cool down bonuses. This really helps any power using class.
