It says that the herbalist gave me a list of ingredients but it is nowhere to be found. Where do I find this list? codex, quest, what?

It's in your herbalism recipes. Like, open up your herbalism skill and you'll see it.

Yep, it's the recipe for Lesser Health Poultice (or whatever it's called, you'd think I'd remember by now).

It's actually called Dwarven Regicide Antidote... or something similar. Making lesser health poultices isn't going to help you finish the quest.
