In ME3 due to a bug or Glitch a Doctor Chakwas during a conversation on the Normady looks like she was in need of a exorcise, Because her head was constantly shaking and the white of her eyes were showing. Picked the other doctor from then on out.

When I found out the second time that Anderson was born in London...

Anderson: You know, I was born in London.

Shepard: Really?

Not awkward but annoying in ME1, every time after Liara taps into Shepard's vsion, cue the " oh I'm dizzy/ you should see Dr Chakwas/ I just need rest scene".
The Miranda ****cam did get old real quick. Once I could let it slide but every scene looks like the programmers are jr high kids.
Every driving part in ME1, I kept waiting for someone to voice over " Damn Shepard- you drive worse than you dance!"
Speaking of Makos, who needs Reapers or cerberus; the way the Mako bounces, rolls over etc, that alone could kill anyone!

Miranda's ass was really too much. I dont understand why they didnt make it normal size. Samara, Tali, femshep and Kasumi all had normal sized ones so why Miranda had a giant inflatable stuffed in her suit?

Ashley sudden quoting of poetry in ME1. It felt like speaking to someone in real life, who says something awkward and you feel an urge to stop looking them in the eyes because it starts becoming embarrassing.

For some reason, Shepard's (M and F) "LET'S NOT DO ANYTHING WE'RE ALL GOING TO REGRET!" line sounds mega awkward.

lol, I think I know what you're talking about. I only played ME1 recently, but I did notice that Shepard has a tendency to shout a lot in tense situations. I think my favourite has to be on Therum.

I was just going to mention this one. Shepard yelling right in Ash's face, Ashley quietly nodding...kills me every time.
