I'm doing my first Insanity playthrough for ME2 as a Vanguard (I wish I'd done it with another class but nevermind). One thing that's really been bothering me is that I can't keep my squadmates alive, especially in the big boss type battles. Right now I'm struggling with the Colossus on Haestrom.
I was wondering if anyone has any tips on keeping them alive, and can tell me how to make them stop following me everywhere I go. It's never really been a problem for me on lower difficulties, in fact I mostly ignored them other than telling them which powers to use, and if they died I could get by alone. But on Insanity, I really need them alive.
The most annoying thing is that they follow me everywhere and seem to move around for no reason, despite me ordering them to specific positions. Due to them having lower health than me, I usually try to keep them back to provide covering fire for me while I move around and try to flank or get up close with my shotgun. But they keep moving out of cover!
Is there something triggering this, or are they just programmed stupid?

Enemies in Mass Effect 2 will target the nearest threat, whether that be you, a squadmate or a Combat Drone. So if you can position yourself in the way of the nearest enemies, the enemies will target you and not your squadmates.
I am paraphrasing here, but I believe I read somewhere that each level in ME2 is designed into sections, so if you move from one section to the next section, your squadmates will rally to your position which cancels your last squadmate order.
You can also order each squadmate to a specific position so you can order them to cover.
Haestrom is probably a little weird since enemies can flank you at times so you have to be a little more careful about squad placement.

Vanguard is the hardest class to keep your squad alive with, and it is because you end up zooming around the map, and they will leave cover to try and keep up.
Basically though you learn where to position them so that they stay safe. ME2 is more demanding with squad positioning than ME3, and to some extent ME1, if the goal is keeping the squad alive.
In general I prefer to stick with the right side for this fight. First order your squad to take cover near Reegar. Then make your way to the right. After you get a foothold on that side, order them take cover at the beginning of elevated walkway. Then clear a path to the sniper's perch. Once you are there, rally them and position them so that they have cover from the Colossus and can engage the Geth below. If you go charging willy nilly they will die. Some people don't care since Vanguard Shepard can do missions all by himself late game, but if you want to keep them alive this is a reasonable way to do it.
Here is an example:

Yeah, I managed it, ironically by going left. The problem I was having with this was that I couldn't seem to get them to stay with Kal, and I was depending on Miranda and Garrus for Overload, especially for the flamethrower Geth. And just to prove my point, Garrus appeared from behind me while I was firing at the Colossus from around the corner, and instead of taking cover behind a crate, he jumped over it and got knocked out straight away. I chose not to waste any more medi-gel.

Yes. That is because there is a limit of distance the squad will tolerate before they try to move to Shepard's location. In fact if you can break the limit very quickly, they will sometimes jump the distance instantly, which can lead to them being way out of position and dying quickly. Hence the fact that the Vanguard is the trickiest class to keep the squad alive with.

Also worth mentioning if you're coming in from ME3, squadmates react about half as fast in ME2 and generally make bad choices with cover and positioning, so you have to constantly babysit them (from ahead) in order to keep them alive.
