It's not going to happen because the DLC can't change the game in any significant way. I say this because nothing from the first two did anything to the main game aside from added war assets and some cosmetic stuff in the cabin.
Will the next DLC kill off a major squadmate?

But perhaps this citadel DLC is a story about Captain Bailey, and in the end of it he decides to join your team, and do missions with you on the Normandy, but then he just dies instead.

Omega had no significant theme of sacrifice, so I don't know where the hell that's coming from. Developers are not going to put content in DLC that discourages players from buying it.
I hope all the people insuinuating that DLC has to have 'an effect' (very subjective in itself) on the main game to be good quality aren't hypocritical enough to be the same people complaining about being 'gauged' for money anytime DLC actually has an impact on the story or characters.

At this stage honestly why would they risk annoying fans even more? Plenty of people are already annoyed enough if they kill off the wrong character that is only going to increase and not decrease this. On the other hand, tears doesn't always mean sad tears /hopeful but not too hopeful.
