Of course, he could return the same as one of the early companions of the PC. Origin companions. Specifically the dalish origin.

To be honest, I don't think it would do very well for the story if he survived. Though perhaps I should say " that he survived as is" Being a ghoul, or worse. Perhaps, alive...but dying.
My argument, on my opinion, is that; Should Duncan have survived, it makes alot of the other elements feel cheaper somehow, and take the edge out of many of the moments in the game in which his memory is invoked. For those who read the books there was a chance to learn more of Duncan, and make the impact of his loss that much greater, those who have only done the game have very little to go by, and I feel that pulling a superman with him would just pull things away from the mood being set. If Duncan's death...hailed as a major event was for nothing, then what does that speak of other events?
Duncan, the way I saw him became the great sagely teacher of classic literary mythos...an Obi-wan if you want a more recent pop-cultured concept... these teacher characters tend to rescue the young hero to be, and build them up, and ready them to complete their epic journey to becoming said hero... it is most often the death of this teacher that finally pushes the young hero to be onto their path...
Now I'm not going to suggest there is no way to bring him back and make it work, I trust the writing staff with that, just as it stands now, I personally don't see it as something that would do much good.

The spell doesn't actually revive the dead. It just heals incapacitated (not dead) party members.

No. Read the codex.
The revival works only if the body wasn't damaged too badly and the spirit still hans't left the body...which it does shortly after the person goes down. Revival is one of the spells that you have a limited time window to cast....minutes, if not seconds.
NOT weeks or months.

Only if it is mostly dead.
