That Salarian special ops leave strategic material/items in wide open areas that need to be hacked, even though the place is crawling with enemies, and has been for the last 2, 5, or 9 waves, but now suddenly they need to be hacked and disabled within a short amount of time.
Enemies repeatedly appearing directly in front of you after you just killed one of the same type in that exact same spot.
Enemies that teleport in, already firing at you. Especially noticeable on Dagger if you stand in the snipers nest window. You can be instantly downed when the wave starts.

A tiny Geth Turret being way more dangerous than a huge Atlas.

You shoot Cerberus Engineer and his turret explode. Why don't the Turret stay there and another Engineer comes to enable it? Disable in the case of Engineer team-mates. It would be cool(??)
That's all I can think of (for now)

When under heavy fire my character will take a relaxed stance on reloading. Unless I "fumble" and try to do something in addition to reloading. Which is the only thing that will motivate my highly trained character to actually use his training and reload fast.
Sometimes my character will just flat out refuse to get into cover and has decided that life wasn't worth living and it would be best to end it infront of random crate of vital toiletpaper supplies.

- Our sniper rifles are what- 4 feet long and our characters can run around with them like it's a baseball bat and aim and fire in a heated brawling battle.
I don't want this changed but it does make me lol whenever I run my SI with a BW. - Turrets have the most advanced AI ever because they have IFF ammunition! They never cause friendly fire for Cerberus but will aim unerringly at you through any gap in cover.
- The way BE points are split/distributed between Adept players
due to who is the initial primer etc. - How quiet a giant Reaperized Space Crab can be creeping in on you..ie the Praetorian. They must have noise dampening anti grav hover units right?
- Killing a seeker swarm reminds me of the old 70's commercials about how many licks it takes to finish a Tootsie lollipop. Yeah- because I can hit it with two BE's and shoot perpetually and really work for that 52 points.
