1. Choices didn't really matter.
2. The ending, EC or not
3. One of the worst journal systems in modern gaming.
4. Dream sequences. Godawful, unskippable, slow-motion dream sequences. They patched the ME2 scanning system to be faster, why can't this be fixed as well? Like with a "skip" button?
5. The story. Specifically in regards to idiotic characters and the horrible Crucible plotline.
Honorable mentions:
6. The EMS system. Specifically the way war asset numbers were just thrown around.
7. The scanner loading screen right before the CIC
8. The lame in-game reason why said loading screen is there in the first place. Two guards and a scanner field to protect against attacks similar to the Collector boarding action in ME2. Nevermind that the Cerberus personnel outnumbered and outgunned these two bored chatterbox guards and their scanning field and still managed to do absolutely nothing.
9. Autodialogue
10. Turret sections. Specifically, useless turret sections that accomplish nothing like on Rannoch and on Earth.
11. The fetch quests. The "Save the Elcor" one stands out as the worst. Clearly, the Reaper "invasion" must have consisted of a single husk which the Normandy's scanning probe crushed when it landed on the planet.
And so forth.

(2) Main story arc incredibly disappointing and unbelievable (crucible and catalyst dialog).
(3) EDI's character development mimicking Data (minus all the charme Data had) + very forced and unromantic LI with joker.
(4) Underdevelopped relationship to any crew-member other than Liara and Garrus (ymmw).
(5) Sidequests solved by planet-scanning.

I'm talking from "in real life" perspective and just kidding around.
From a ME perspective I'm not a particular fan of Tali, so it's not a big issue to me I suppose. I always imagined them as dark skinned, glowing, white eyed humanoids... so it didn't hit me that hard: only that I imagined them being more ebony than purple.

And I guess Omega as well, that was a missed opportunity for Nyreen as a squad mate and a new hub. I still love Mass Effect 3 though, and I love the extended cut endings.
Edit: Also War Assets are a missed opportunity as well, I like the idea but it was executed awfully. And the journal sucks as well.

2. Fetch quests and poor side missions in general. It would have be awesome to walk around on Feros again.
3. Lack of time spent on species homeworlds. I wanted a hub on Thessia and hell we never even got to Palaven.
4. The diminishment of previous decisions that were could have been huge. Like the rachni.
5. The weight system.
I still do enjoy ME3, but it just lags behind the previous installments.

ME3 on the other hand is a shooter wiith a few RPG elements, it's a really good shooter mind but compared to it's predecesors.it's a step backwards in almost every possible way

1. Lack of Harbinger:
When you are escaping the Collectors base on ME2, Harbinger claims he will find another way and the game ended with a "Harbinger is coming" line followed by a scene with him leading a massive Reaper fleet. This established Harbinger as the leader of the Reapers and the main antagonist. But in ME3 he is pushed into the background, with Shepard having no interaction with him of any kind and while his entrance when he lands near the Citadel beam on earth and the subsequent race to the beam are fantastic, he just leaves the game after that (why he left Shepard's body behind after wanting it so badly during ME2 is a mystery). Having TIM being possessed by Harbinger during your conversation with him would have been great, even if it ended up with TIM killing himself after gaining some control. Hell even having Harbinger haunt Shepard during the dream sequences would have been nice.
2. Kai Leng:
Instead of Harbinger ruining Shepard's plans at every turn we get this guy, a lame one dimensional villain whose "boss fights" are as cheap as they come. He adds nothing to the story and in your interactions with him there aren't even dialogue options. He should have died on Thessia with Shepard climbing up and gutting him with the omni-tool right there. Nah, scratch that, Thane should have owned him on the Citadel.
3. Lack of Mako or Hammerhead missions:
Missions such as Project Overlord in ME2 where you switched between doing parts on the Hammerhead and parts on foot were fantastic. Missed something like that on ME3.
4. The Journal:
It was a step down from ME2 as it lumped together all types of missions into a single list and it was much less informative on what had to be done on each quest. On the PS3 (not sure if it applies to other systems) it also has this bug where you always have to scroll up to see the top of the list as it always starts half way through the list.
5. Scanner loading screen on the Normandy:
It is quite tedious as you have to cross it after nearly every single mission. I don't get why it is even there to be honest, the Commons and the Docks Holding Area of the Citadel have far more content in them than Deck 2 of the Normandy and don't requiere such type of loading screens.

3.Suqdmates from ME 2: needs more attention. 1 mission for eveery1 of them and no more speaking or messages from them 'till last mission.
4 Autodialogue and no really impact from our decisions. (save/kill councils, rachnis (?), collector base)
