Let's be honest, we all made some dumb choices in our first playthroughs that came back to bite us in the rear in ME3, and while we went back and fixed what did wrong, we all screwed up our first few playthroughs. So what decision do you regret the most?

For me, selling Legion to Cerberus. I had no idea that he was a friendly Geth (after playing insanity in ME1 can you blame me?) and I was seriously strapped for cash. Well, no peace between the Geth and Quarians for me, sorry robot bros, but I coudln't betray Tali.

I read a walkthrough when I played for the first time. I being on PS3 started on me2 and I had everyone survive the suicide mission on my first playthrough. I just got mass effect ,and I'm doing it without a walkthrough. I haven't seen any mistakes so for ,but time will tell.

Well, I cheated on the suicide mission because ME2 was about a year old when I first got it, so I got to say my worst decision was letting Kelly keep her identity on the citadel. Didn't see her execution coming and it hit me out of the blue. I really liked that poor yeoman.

Honestly there are very few decisions to regret because most of them didn't matter

I think the biggest mistakes would be selling legion and killing samara, but thats due to "laziness" in design than actual consequences

Not saving the Genophage data....Eve death was very disappointing, I liked Eve....I did enjoy Wrex's reaction on Surkesh though.

Trying to get Miranda killed(while promising myself not to reload) and getting Legion killed on accident instead. Yea...sorry about that Geth, I really didn't mean to doom the peace...oops.

I made two mistakes in my first ME3 play through. First was telling Kelly to keep working at the shelter which got her killed, and at Mars not using the "investigate" like response to the Illusive Man which barred off the Paragon/Renegade choices with the Illusive Man endgame. I can say just about everything else played out perfectly

I regret no choices in all three games. From RP perspective I made all the right choices all of the time in all three games and they all turned out good to me.

I think I'm luckier than most in that I never killed Wrex or lost Legion or anything like that. No regrets from ME1 or ME2. Unfortunately, in my first playthrough of ME3 I forgot to meet with Miranda on the citadel the first time, so I never got to warn her about Kai Leng, and she died. I also told Kelly to help the victims instead of changing her identity, so she died too. Those are probably my two biggest regrets.

Edit: now that I think about it, it seriously irked me when I bugged out that documentary quest on the citadel by accidentally taking all of the bad photos first, which makes it impossible to complete the quest.  Being a total completionist, that was super frustrating, and I never would have let it happen had I known the quest was bugged like that.  Regret #3 =P.

My first playthrough of each game is as follows:

ME1 (back when I first got it for the Xbox):
-Colonists dead on Feros. I was pretty dumb when it came to the inventory so I didn't even bother equipping that Thorian gas mod.
-Wrex dead - didn't have enough charm
-Rachni dead - I thought she would betray me if I let her go. I wasn't even renegade.
-Council dead - I thought Sovereign would win if I didn't hold the fleet back. That was before I learned that our decisions don't really affect the progression of the plot.

ME2 (with the previous decisions above):

-Attempted all of the loyalty missions.
-Failed Thane's loyalty mission. I couldn't update him in time.
-Failed Samara's loyalty mission. I couldn't seduce Morinth in order to get me into her apartment. Reaaally embarrassing moment for me. I got rejected by a fictional character, great.
-Lost Jack's loyalty during the argument she has with Miranda. I was able to win it back when I got my Paragon up.
-During Tali's trial I gave the Admirals the evidence which made me lose her loyalty.
-The Suicide mission was a disaster. Half my squad croaked.

I never got to import that save into ME3 since I switched platforms. My first ME3 playthrough was on PS3 with a "perfect" Paragon character.

-Tuchanka Bomb went off.
-Kelly died.
-I didn't talk to Thane so he didn't show up to save the Salarian Councilor. And since ME1 wasn't on PS3 back then Kirrahe couldn't save him either.
-I didn't have to kill Kaidan during the Citadel coup but he didn't trust me enough to join back up with me on the Normandy.
-I didn't have enough reputation so I couldn't get the peace option on Rannoch. The geth died.
-Miranda died. Didn't warn her about Kai Leng
-For some derpy derp reason I didn't save Anderson from TIM.

I regret not clearing Tali's name in ME2 for my first playthrough. Ended up losing the entire Quarian population in ME3.

Destroying the Collector Base.

I was stupid to think Cerberus would become some sort of unstoppable force we couldn't take it back from.

But perhaps I was accounting for BW's over-the-top writing that would have made it possible.

Hmm... My first and second play-throughs were pretty annoying.
Wrex dying both times, Jack dying the first time and then Miranda dying second.. It took me a while to actually manage to keep them both alive. And of course Tali getting exiled I did everything right the first time but the second time round and nope It just wasn't happening no matter what I tried or how far back I reloaded (and I did go at least 3 missions back). Finally- picking the Geth and losing the Quarians... I didnt know there was a way to save both.
I did both play-throughs practically the same aside from getting things I missed the first time and trying to keep those who died alive yet both play-throughs sucked.

Rewriting the Geth Heretics - Not really as big of a deal as some of the other stuff listed in this thread, but it does sort of backfire on you in ME3. If you rewrite the Geth the Quarians take heavier casualties before you can negotiate peace, and you end up with less war assets than if you had destroyed the Geth.

I also told Kelly to keep working with the refugees, and she got shot in the face during the Coup.

Miranda also got killed by Kai Leng, because she had been the LI in ME2 but I had Shep go back to Liara in LotSB (played after the Suicide Mission) and ME3. I wouldn't necessarily call that last one a mistake, but it was a decision that ended up having severe consequences.

Not clearing Tali's name. If I had known she could survive without being loyal, I totally would have done it. Paragon Shepard wouldn't have wanted her exiled, but he gave in to her pleading despite not wanting to.

Not a mistake, per se, but paragon Shepard would have preferred stopping Garrus in his loyalty mission. He doesn't regret it, just thinks it would have been better.

Not bribing/intimidating the Citadel shopkeepers before buying out all their stock, then finding out I got a discount, does that count?

Yes, same for me...I got Kelly killed...pretty much the one thing I did wrong...

My Defualt Shepard would be really disappointed in you for selling Legion.
With Legion and Tali, that equates to 2 widows, 2 Combat Drones and 3 bad asses on the field.
Insanity was a cake walk.

Not a lot of people get this one, but saving the salarian Spectre in the Hanar Diplomat mission in 3. It's not a problem if you have Kasumi, but if you don't then it's a choice between him or the entire hanar homeworld.

He seemed like a cool guy so I hit the Renegade interrupt to save him on reflex and didn't realise that I'd just doomed a planet. This was from a mostly paragon female Shepard who romanced Thane as well. Ouch.

Interesting! I had no idea that scene played out that way if Kasumi wasn't around.

Sad part was I didn't even realize he was an ally until I saw my brothers playthrough, "What's with the walking flashlight?"
