I had Miranda as my biotic sheild in the Suicide Mission Causing Garrus to be taken out by the swarm, also I did Zaeed Loyalty mission first, being a paragon couldn't talk him round so he died holding the line.

ME1: Ashley shooting Wrex due to lack of charm.

ME2: I had already lost Jack, Thane, Legion and Jacob so I was worried something might happen to the biotic specialist, so I went with Miranda because she was my least favorite of her and Samara, whom I really liked, so I lost LI Garrus AND second team leader Samara. Facepalm. That suicide mission was a disaster.

ME3: No regrets on first playthrough.

Forgot to mention, never being fully renegade or paragon on my first playthroughs. Couldn't talk Wrex down, couldn't solve the argument between Miranda and Jack (lost jack on the suicide mission). And going out of my way to help Cerberus in ME2, I didn't have many friends after ME1 and I bet on the wrong horse.

But I will always stand by my decision to beat up Al'Jalani, I don't care if I miss out on a War Asset because of it!

Getting Kelly killed in ME3 my first playthrough because I did not tell her to change her name.

Don't even have my original ME2 playthrough any more so my mistakes there didn't mean much. I lost Legion in the tube though (didn't have loyal Miranda and assumed that she would be professional enough, as well as concerned with her own survival, to do the job as well as possible) so that would've had a rather negative impact on ME3.

The worst decision I made was to play ME2 before ME. All those moments that would've been so awesome if only they'd meant anything to me. How I wish I could've known who the hell he was when Archangel was revealed to be Garrus, instead of my just thinking "Oh, right, it seems like I already know this guy. What's happening next?"
