While running through the Chantry during Act 2 I noticed a girl standing by the door. She was wearing robes like Elthina with a crown and cloth that covered her face. She appeared to have a hand-maiden standing next to her. Any thoughts on her identity?

I don't know exactly who she is, but she appears to be a random Chantry sister. On occasion she'll speak when you walk by and say "Many come to the Chantry to find succor. What are you here for, messere?"

The lady beside her, the one in plain clothes, is actually talking to the Sister about wedding plans for her daughter. At certain times, you hear her ask the sister about decorating the church, and the Sister then asks the woman if it wouldn't be better to plan a wedding when the girl actually finds someone to marry. The woman responds "Details, details!"

I've often wondered about that woman's head-dress and why it covers her eyes. I theorize she's actually blind, though I'm not sure if that's true.

Makes sense. I thought she was someone important who just never said anything.
I ran by her, turned around and stood there for a moment and thought it was Divine Justinia V in disguise.

It's just a headpiece with a thin cloth over the eyes. Probably just new Chantry apparel for DA. You can still see through it. 

Enchanter Ilana wore the same thing.

Oh yes, I remember the wedding chat from my playthroughs. Never paid much attention to it, sounded like some noble planning a wedding before she finds a fiance.
