Iorveth was confirmed in one of the interviews.

Considering Saskia has a very large character and you can also decide her fate in Witcher 2, it would be silly not to bring her back.

I hope Shani will appear, i really liked her in the first game.
Letho and Geralt need to team up, i really wana see these two witchers working together.

Is this game a concrete 2014 release or what?

I find it hard to believe a game that's supposedly much bigger thant W2 will be out in 2014.

^ yes, 2014... with Cyberpunk 2015 release date.

PS: they are probably started making it after PC release of W2.

I tried the "Lykaon" mod for TW2 (it's an alpha though). It was really, really good. The writing and the setting are perfect. There are a lot of issues (enemy spawn, optimization...) but I'm looking forward to the final release of it.
I really hope the mod community for TW2 will be huge. CDPR deserves it.
