Sustained spells/talents keep being deactivated

Google, duckduckgo, etc. provided no answer to my problem, so I'm asking here: Why do sustained spells/talens keep getting deactivated during ombat?

I'm playing a Blood Mage (currently lvl 12; playing on Xbox360 btw), and there are three things always activated: Blood Magic (duh), Rock Armor, and Summon Dog. But during almost every single battle, all three spells are being deactivated at once without any apparent cause. Sometimes this happens right at the beginning of a battle before I can even cast a spell or before Hawke even takes damage. And I didn't assign sustained spells to the combat menu, so me turning them off accidentally isn't it.

If anyone can provide a solution or at least an explanation, that would help a lot. It's currently really frustrating to play the game. :(

i looked into this before, if i remember it could be caused by set bonuses on gear namely "chance on hit" to stealth, whether or not the rogues talents that do the same are also guilty of it i dont know.

What i do know is i have the problem on my warrior an she is wearing an assortment of gear with no set bonus an no bonus to on hit = stealth, and i also tried soloing with her but to no avail the sustains always kept falling of, got that bad ive never played her again

In the meantime I've used a few Maker's Sigh potions (and lost attribute points in the process every single time) to build mages with different specialisations and sustained abilities. All of them get turned off as well, doesn't matter what it is.
