Has this ever happened to anyone else?
I just find it odd that she went from not knowing my location and doing random melees to locating me on the 2nd Shadow Strike before my attack even occurred.

I go for duration on cloak, don't have melee weapon attachments, use a 6/6/6/0/6 build, and don't typically fire my weapon before hand.

You should ahve melee weapon attachments if you mean to use SS as a killing power. Otherwise, you're losing a lot of potential damage with her.

Working as intended. The enemies will always see you when you're that close. And never approach a phantom after she's melee'd or you will get sync killed.

I'm aware of the risks of Shadow Strike (learned the hard way lol). I've been using the Shadow as my main character for quite some time. Phantoms are typically not an issue with my build. (I play on Gold.) However, during my last match, I SS'ed a Phantom, dodged back, it started randomly melee'ing, and coming in for the second SS, I got executed with her sword kill. It happened before the sword attack on the SS. Never seen it before (not in my matches, nor in any gameplay videos I've seen).

If a Phantom (Or any other Sync Kill unit except Banshees) does a melee attack, then anyone in their vicinity is open to a Sync Kill, regardless of what they're doing.

Shotgun blade attachment is a common mod, why are you playing Gold if you don't even have your commons maxed?

Okay here is something important: Both Biotic Charge, Shadow Slash and Havok Strike place you beside the enemy a good while before the attack actually connects, that is particularly visible the higher your ping is as the enemy has more time to react to you, I presume by design there is not supposed to be too much time between you being there and the attack connecting.
What happened here is that the first SS did not kill it, the Phantom meleed you and missed but this set her up for a sync kill. You used SS again, the game puts you right beside her, she's still locked on to you and since you're beside her using a power she can hear you despite the cloakn, since you're detected and in sync kill range, she syncs you during the delay/windup for Shadow Strike.
It'd be advisable not to attempt that maneuver again like so.
