She was my favorite character! I would've gladly been hung in her gallows to let her reign with an iron fist! (Didn't care much for the Hawkes. Hated myself the entire time. My voice. Personality. Glad I could kill that sister of mine.)
Was there any way to save Meredith?!

No, Meredith was lost the moment she came into contact with the idol.

To be honest she didnt seem THAT crazy until she called for the Rite of Annulment. Even before the explosion, she actually seemed to be revealing more of her inner self:
(Orsino saying something about Not all mages are dangerous, and that Meredith shouldnt generalise that all mages have to be watched over 24/7)
(To Orsino) "I know, and it breaks my heart to do it, but we must be vigilant. If you cannot tell me a better way, do not brand me a tyrant!"
Dialogues such as this. At that moment, it seemed like all she was doing was because of some divine "duty" she had to do (protecting kirkwall, or whatever. Or it could be the lyrium idol.)
