Mostly, I try to name them the way their race dictates... such as Markus Bharilla the Turian Sentinel, Gormack Duraan the Batarian Soldier, etc.

I give them names. Real names that fit their race/gender.

Here is how I name my characters:
I imagined each class as its own squad.
Each squad gets a callsign, ala military style, or when you hear Cerberus bantering in SP.
Each member of the squad is designated with a roman numeral within the squad.
Adept - Osiris
Soldier. - Raptor
Engineer. - Hydra
Sentinel - Wraith
Infiltrator - Echo
Vanguard - Ghost
So:
Salarian Infiltrator is Echo III
Human male Novaguard is Ghost I
N7 Paladin Sentinel is Wraith VII
Battlefield 3 Soldier is Raptor V
You get the idea.

I get really, really, really, really creative.
Human Male Solider = HumMaleSold
Rinse, repeat ad nauseum.
Really, what's the point?
I'm the only one who sees it, why do I care?
Sorry, this is one aspect of the game that totally eludes me, my whogas factor approaches 0 lim.

"Summit" - Male Vanguard
"Peak" - Male Infiltrator

Oops double post!
I also gave my geth codenames that mirrored 'Legion' in the sense of multiple entities acting as one, eg. Unity, Army of One, Phalanx

My kits' names:
Adept: Imperator, Steph, Hesper Armali, Tydes, Nymandra*, Marcus, Egypt, Byodic Aud (get it?), Proteus and Boerwen
Soldier: Erik Deacon*, Shel, Hurc, Torvan, Cpl. Ben Herman*, Bahttur (as in "swing..."), Urgkha, Invader, Flyover, FAU_1109 (for Frontal Assault Unit) and Garrin'haard
Engineer: Implementor, McCreary*, Aizee'hott* (cryo + incinerate, get it?), Pardik Hallis*, TSU_4077 (for Technical Support Unit), Shaokkn'hott, Kat, Volvott, Tyrnus and Kolchur
Sentinel: Interdictor, Theresa (after my mom), (also) Interdictor, Tossurgh, Katchmin, Faulhaut, Cmdr. Gallagher*, Nurf Urdhur, and Valence
Infiltrator: Intervenor, Yce, Querny, Ova'rayyed, SRU_1018 (for Stealth Recon Unit), Ject'ive, Zen, Aerraed ("air raid," hah!), Seiral Kier and Shae Valthrace
Vanguard: Hadrian Shepard*, Hellen, Thorne Korpis, Ashalt, Ooraah, Holden, Chen*, Tur Fedoe and Churjur
All the *s denote character names that I lifted from the post-ME3 fanfic I wrote (and the sequel that's just about finished), so when I play those characters I sort of indulge in the notion that I'm playing out those characters' activities during the war, so when I write about them I can imagine all the battles they've been in
Steph's my biotic battle goddess, to play her I imagine a telekinetic goddess Athena, or Jack but with a calmer, more controlled demeanor-- stoic except for taking exalted joy in blowing things away with her biotic heavy melee or BEs.
And the In/Im's (Imperator, Interdictors, Intervenor, Invader, Implementor) I all imagine as being graduates of an "Imperative Missions Advanced Battle Tactics School" so they're my ultra-elite 'no fear' mindset characters. Bad guys don't see their backs because they leave nothing alive behind them
BTW-- news flash, I'm a big RPG nerd.

My N7's are named after a group of characters that draw/write about in my own personal stories. My Krogan Soldier and Sentinal are, Abba and Depeche. My Vorcha are named after Mexican foods (to an extent); Churros, Tortilla and Taco con'carne. Most of my other characters are named randomly, but attempting to sound "alien" and lore friendly, Quarians with their "vas" and "nar" and such.

I just think of random words on the spot and whatever I think of becomes the character's name

If I remember correctly, one of her Asari character's name is Tenpickles.
I happen to take the nameing of my characters seriously.
Volus Adept = Sir Hairyballs

My One True Save File Shep for the whole trilogy was named Jenny, so all my female MP characters are versions of that - Jen, Jenn, Ginny, etc.
Males and non-gender-specific are named pretty literally - for example, Batarian sentinel = Bats, Quarian male engineer = Quarie.
I really only had fun with my volus and vorcha. The volus adept is Cal after Cal Niftu the Biotic God, the vanguard is Sonic, and the first vorcha I unlocked is named Mr. Ambassador because of the Blasto easter egg in ME3 (deeplomatic eeemyoooonityyyy!).
BTW Derek, I love not having to rename my characters after each promotion. I know that feature's been around for a while, and it's a small thing, but I appreciate it every time I promote.

I tend to opt for ironic and non-serious names usually based on their class and power set.
So here are a few of my favorites
Human Adept - Husk Herder
AJA - WannaBeABanshee
Phoenix Adept - Slap Happy
HumSol - HumansRSuperior (cerberus-esque colors)
GethSol - Cylon Toaster
HumEng - Walkin' the Dog
N7 demolisher - Michelle Bay
KroSent - Eats Hot Lead
GI - HK-47
N7 Shadow - ImmaCutYouSoBad
Humguard - RedOnesGoFasta! and Pinball
Kroguard - Mack

For vorcha, I mash my head on the keyboard and see what I get. I figure, if they had a name they probably couldn't spell it, or if they could, that's how they would do it.
This is by far the best and most creative way for naming I have seen yet. I love this.
