(1) When characters have sexed-up outfits at the expense of protection in combat or in hostile environments. The worst example is Jack in ME2.
Those armor complaints could of been easily avoided if Bioware added armor to each character while in combat situations or space, while still keeping it unique (Miranda's DLC armor for example, or Jack with some scarred up graffiti light armor would of been epic), but I guess it wasn't a priority at the time.
That's one reason why I never shut up about things like this: it's easy to avoid.
Also, I'd like to add that I find a character off-putting if her appearance is like she's carrying a big sign screaming "I AM SEXY! STARE AT MY T&A". All too often, that makes her appear as if she's generally available, and I do not find that attractive. Generally, this happens when characters wear things like skintight catsuits in situations where drawing attention to sex appeal is not socially appropriate. Like, say, on the bridge of a military starship...
