During my first play through of the entire game - deffo deciding between Ashley and Kaidan in that first play through as I liked both of the characters and did not want to kill either of them. However Kaidan won because he was my characters LI and Ash died.

For me it's probably dealing with Balak in ME1. I typically let that weasel escape. And I loved the renegade response Shep gives the engineer when he asks why you let him go:

"You ever go hunting? Hunting requires time and patience. Balak will be back. He'll make a mistake. And when he does I'll come down on him the like the hand of God."

Likewise when you meet up with him on the Citadel I always take the renegade option, basically daring him to kill me, the only hope his people have to survive, rendering him my impotent tool.

There were actually two times I reloaded a save and changed a decision because it was so hard.

1) Destroy or rewrite the Heretics. At first I rewrote because I was a compulsive paragon, but after I thought about it I went back and destroyed them because I felt either way it was the morally right thing to do.

I also had a lot of trouble deciding whether to save Kaiden in a femshep playthrough, I ended up going back and saving Ashley instead.

I had two playthroughs where I arrested him, one who left him to die, and one who did what you did and then made good on the promise to kill him in ME3.

Hardest choice for me is deciding to ever try and play ME3 again.

Actually, scratch that. Deciding what happened after ME2 via headcanon is definitely the hardest.

Probably the genophage (with Wrex, that is; with Wreav it's an obvious sabotage). I'm still not sure either way on that one.

The ones that others seem to find the toughest, Rannoch and Virmire, weren't difficult at all for me.

Honestly? Whether to give ME3 one more chance and attempt another playthrough.

In game... The ME2 genophage choice...whether to save or destroy the cure. I didn't think the Krogan deserved to still carry the genophage but on the other side I saw what could happen if it were cured. I decided to save the cure in the end, figuring that it's still not a cure. Wrex made me decide to cure the genophage in the end.

ETA: When taking into account only the first playthrough: the Virmire choice. I think I stared at my screen for at least 10 minutes before deciding who to save.

These were my toughest choices on my first playthrough of each game:

ME1: Virmire (I knew what was coming and I thought about it for the whole game up to that point. I eventually chose to save Kaiden)

ME2: Whether or not to keep the Collector base (I eventually chose to give it to Cerberus. Big mistake)

ME2 (First Renegade playthrough): Deciding whether to kill Morinth or Samara

ME3: Red, Blue, or Green explosion (I think you get what I mean)


Most of these choices have gotten easier, but I still have one decision that stumps me for a bit:

Play ME3 MP or continue current trilogy playthrough.

Gets me everytime I jump on the game. It's even harder when my current trilogy playthrough is in ME3.

ME 1: Virmire is emotionally difficult but intellectually and ethically it may as well be decided with a coin toss so I'll go. Probably Balak although I don't really see that getting one guy is worth the lives of several others (get rid of one criminal and another pops up to replace them anyway).

ME 2: What to do about the Heretics. The geth are too different for it to be at all valid to regard them in the same way as I would having to make the same choice about a person. Trying to get an insight into the geth was probably the most interesting part of the whole Mass Effect trilogy, a truly alien mind that isn't just pretending to be so by being random or arbitrary yet also seems entirely valid. When science fiction delves into that sort of thing it's at its best.

ME 3: There weren't any, and I'm not just being snide here. Geth / quarian would've been a very strong contendor if I hadn't had the chance to make peace. Genophage would've also been an interesting one if I'd got Wreav instead of Wrex to deal with.

The choice between Kaidan and Ashley on Virmire.

I didn't expect it at all.

By ME2 and ME3 I was expecting tough choices so none really came as a surprise or was difficult. Even the ending one was simple for me.

Whether or not I could trust Miranda with Alliance resources (without knowing the result of that decision). Between Udina's betrayal and Dr. Eva infiltrating the science team on Mars, I had a difficult time believing Miranda cut ties with Cerberus, considering how loyal she was to them in ME2. I didn't even know you could get her to resign in that game until yesterday. In the end, I decided it's better to regret trusting someone than to regret not trusting them.

Geth/Quarian conflict solution missions were the best for me. I am a Talimancer, and also I find the Geth to have gotten the most unjust fate. Almost made me cry when legion showed me "What did we do wrong?" in the consensus..

Quarian or Geth is a big one.

I forced this choice by not saving Admiral Koris. I had actually planned on that playthrough not to make peace because I wanted to see what the choice was like.  I read about this on the forums and watched the video, and I figured I could deal with Legion's death ok. But I didn't realize there was and EDI conversation afterwards, and when I had the conversation I was, like, "Wow! That just provided a whole point of view I wasn't expecting. Wait, why DID I pick the Quarians over the Geth?" I had never even considered picking the Geth over the Quarians, even though I consider the whole conflict the fault of the Quarians. I discovered I'm a complete pro-organic racist:?.

ME2's Heretic rewrite choice is another one - kind of parallels the Destroy or Synthesis choice. I spent a good five minutes considering that one the first time.

ME1's capture Balak or save the hostages. Its the one choice in ME1 that can lead to hundres of people dead in ME3 due to Balak being on the loose in the Citadel and committing terrorist acts.
