So, I just killed the Archdemon.. yada yada...
43 something hours played, and only like 53% of the game completed across multiple play throughs. What could I possibly be missing?

There are several possible endings, depending on your origin story and who you decide to put on the throne in the end.
Do you have the Stone Prisoner and Warden's Keep add ons? That is more content and more areas to explore.
Like Bas said above, the achievements are your key. Some are "Secret achievements" and some are more known.
