If you're like me and like going all out paragon/renegade, and like having a few scars but don't like accidentally slipping into glowing red eyes, or seeing your scars suddenly vanish. Here's how you can manually keep yourself
"Scarred for Life"

This will require you to modify your save game para/ren points with Gibbed ME 2/3 Save editor, and only recommended for NG+ playthroughs. I'm not gonna tell you here how to use this editor, it's fairly straightforward and google is your friend.

These tips work for ME 3 and ME 2 (values may be slightly different here though).
I haven't had any problems yet, but always excercise caution:
ALWAYS BACKUP YOUR FILES BEFORE MAKING CHANGES.
Gibbed is really really good, but you never know when the universe might explode.

Let's begin then:

Note that these values are really big because you get about 500~1000points (might be more in ME2, can't remember)
playing as a purist, and depends on what and how you do things ingame.
Having a big number means that the balance doesn't shift all the time, and should only change after several playthroughs. And the key to constant "unchanging" scars is to keep the values balanced.

Assuming Paragon / Renegade points here:

These work well for someone who mostly does renegade stuff, but if you're going for a scarred paragon you will need to adjust the paragon values a bit. Otherwise your scars will at some point start fading away mid-game.
Adjust how you want things to be, these aren't set in stone, just my own personal preference.

No scars:
All you need is lots of Paragon points, about 90% should be safe enough.
Or just use the ingame healing option to get rid of them forever.

Basic scars:
The first scars you'll see will start showing when you do a 10k / 4.5k split
and will last up until 10k / 8.5k, which is about the time you get a few more scars to look at.

If you only went with 1k / 1k, you would start getting glowing red eyes at 1.5k Rpoints, and thus breaking the balance.
We want to keep the Renegade score low enough so it doesn't go above the "limit" too soon no matter how renegade your Shepard is. This in turn means that you should be able to do at least 4 NG+ playthroughs before reaching the 8.5k boundary.

A few more scars:
You can start at 10k / 8.5k, but unfortunately it only last til about 10k / 12.5k at which point your eyes will start to glow red, but should remain constant for 4 playthroughs.

3 Glowing Dots:
The 3 glowing red dots in your eye iris will start to appear at the 10k / 12.5k mark, and these last for quite some time.
As far as I've tested this should last to about 10k / 23k. Now that's alot of NG++

Glowy Terminator Eyes:
Lots of Renegade points is all you need. Keeping it at 90% at all times should be good enough even if you decide to do a few paragon choices.

End notes:
That's how the scars basically ingame work, and you can adjust these values however you like them.
I don't know if there's a limit on how high the numbers can go, but so far I haven't had any problems with these.

Even bigger numbers like 10M / 10M could possibly work for NG++++ to infinity and beyond, well almost, and then you could practically forget about them after that, but I haven't tested if the game allows such high values.
But if the universe explodes just reset them back to something normal.

Hopefully someone will find these tips useful in becoming "Scarred for Life".

vice versa for me, I like the scars but not the red eyes
without using such tricks it was some back and forth before I got the amount of scars for my renegon Shepard right in ME2
;)
