It's happened on two different playthroughs, when  get to Dagna I go to the Tower and after I talk to irving and then go upstairs to finish the deal with Godwin on the Lyrium trade I head back down and as I approach the Apprentice rooms I notice Dagna is in the tower, I talk to her and then it send me to the area just before the stairs to the next level. However I still finish the quest.

There is also a problem with the Chanters having exclamation points above their heads or so forth, randomly in Redcliffe I have no idea why but it won't let me talk to them as though they had a quest for me or such. I can talk to them if I do actually do a quest though and that usually gets rid of the problem. But it's annoying to have it occur.

I've also run into the first problem you've described. Imagine my surprise that Dagna was already there! But then I went back to Orzammar and she was there too! Maybe she has a twin

So it seems though I notice even more problems liek on my newest file in the Dead Trenches I notice that I kileld the spirits to get the legion key and they won't vanish worse yet is they sparkle like I can ransack their corpse .-. yet it gives me no option.
