There are three, non-mutually exclusive ways one could go about this:
1.) Raping the player character.
Let's say your character, the Inquisitor, is forced to marry a very wealthy and influential trader or nobleman/woman. That person then, commits the Inquisitor to a brutal, abusive, and often humiliating spousal relationship. This sort of story hook could easily provide the player with a strong reason to follow the quest up until its culmination, resulting in being given the choice of either killing your torturer, or sparing him as he pleads for his life.
2.) Have the player character rape someone.
It could be revenge, or merely compensation for a service the player character provided by lending his sword. Some would argue whether this qualifies as rape at all, but if such an NPC would be initially unwilling, yet circumstances forced him or her to give sexual compensation as the only reward of choice, it could still qualify as rape in most countries, even if the character eventually (but reluctantly) agrees to it.
Alternative: Raping one (or more) of your companions. This could have a profound impact on the game's story.
Think about it. How will that person view you? Would (s)he try to remain stoic in front of the horrors she endured or would (s)he suffer a mental breakdown, and no longer be of use to your character?
More than that, if your companion(s) cannot contribute further to the party, how will they be dealt with? Will the player character kill them, abandon them, or just use them nothing more than sex dolls -- something to be thrown away?
3.) Force your companions to rape someone.
Your companions could be rapists. BioWare has had evil companions accompany your characters in the past: thieves, murderers -- so why not rapists? These companions could be glad to serve under the Inquisitor's command, or they might be reluctant to do so; even more, the Inquisitor would have to watch his/her back around them!
