So I've played DA 1 and 2 a lot lately, and I've been thinking about the amount of people you kill in each. In DA 1, you do kill a lot of (non-darkspawn and non-demon) creatures, but it's nothing compared to DA 2. On my average walk through any given section of the city at night, really large groups of people attack me on a regular basis in waves.
In DA 1, I didn’t always feel like killing people was
meaningful, but it felt a bit more important when I did on average than when I
did in DA 2. At a certain point I was wondering if I had killed off a
significant portion of Kirkwall’s population, but this never seems to be the
case as the sheer volume of attackers never seems to recede. I say this, because
during different acts of the game you have to clear out the same portions of
town than you did the first time.
I suppose it’s reasonable to say some of the people were
replaced by say, Fereldens or some other migration of people, but lets just say
you clear everyone you can in a specific act. All the bandits, all the thieves,
all the random encounter attackers, blood mages, etc. Kirckwall’s population is
still pretty set.
But that’s not what really bothers me. What really bothers
me is the fact that when you kill that many people is that no one seems to give
a ****. I just killed… at least 200 people possibly 300 (if you take Aveline
then you’ve been doing it in front of the guard), and no one seems to notice.
Most of all after a while I just get used to it. I kind of
get desensitized to the mass murder of all these people, regardless of whether
they deserved it or not. I don’t even feel like I’m helping, even with the
reward you can pick up for it. It’s become a fact of life; I walk into the city
at night and there will be random thugs, a never-ending wave of thugs that can
never be totally destroyed, because they’ll just be back in a year or three.
Even the times where you “magic murder knife” people in
options is kind of lame compared to DA 1. In Awakening, for example, I was able
to kick a man out of a cage and into lava. In DA 2, I guess I get to shank
Aveline’s husband and that’s cool, but it’s like whatever honestly especially
if you’re a mage. I mean, he was already dying, I can’t claim to be an ****
if there’s a moral justification.
That’s just my feelings on the matter. Would be cool to see
what other people think.

I think this is a case of Varrics lies. In truth you were only fighting a group of 1-5 people, 10 if you had a busy night. Cassanda did ask Varric to tell her everything so instead of telling her "And then Hawke and co. met a small, malnourished robber, branding the tiniest dagger in his shaky hands...." he decided to splice things up a bit.

Or Kirkwall is just ridiculously big and the population can handle the massacre of three large gangs (+around hundred other casualities) every three years.
Or maybe they ship in mooks from Ferelden, Tevinter and Rivian (got to keep that crazy Hawke busy lest he starts attacking the civillians).

And finally, about Aveline. She doesn't complain because the guards are a ten times worse. Seriously, in Act 3 when you go to check up on Donnic, he's slaughtering a tiny army. It's a tiny warzone out there and when the corpses have fallen he doesn't even thank Hawke for pitching in! Aveline doesn't even ask if he's ok! They just act casual, as if slaughtering a group of around 50 people is just an every night occurrance. So seriously, she has no right to complain about Hawke.

The whole of Kirkwall is actually just bandits. Hawke and pals are, unknowingly, the only non-bandits in the entire city, which is why they are ambushed so often.

It's worth noting that a large percentage of the people Hawke slays are foreign mercenaries and slavers. Nobody particularly cares about their deaths.
As to the rest of it … Kirkwall is a pretty dangerous city. But really, is it any worse a fictional hellhole than, say, Arkham City? I just did a playthrough of Arkham Asylum wherein Batman shattered the arms and legs of dozens of mooks. Will filling up the city's hospitals with injured thugs have any actual impact on crime? Not really.
