I got Shepard helping an invisible Rodriguez on Grissom once. The Glyph bug is really annoying, and Aethyta still thinks Shep and Liara are together (though this worked correct the next to last time I played, strange).
They REALLy should do something about the Rannoch rock.
I've also had the Save Victus's son mission totally breaking, so that I've had to restart it.
I would also be forever grateful if they made the awesome Leviathan music work in the game.

Better questions is why is Ashley wearing make-up into combat, a catsuit and have her hair down in ME3 when in ME1 she's the complete opposite and would never do any of this stuff.
