Holy god,  you just made me ROFL.

1) Space Hamster Indoctrinated. The reason for Shepard's nightmares revealed! "He Watches You, Even When You Sleep". 
2) The Bioware staff. "Revenge for the Fallen" (double meaning)
3) Harbinger indoctrinations. "All Your Citadel Belong to Me"
4) Urdnot Rex breaks wind which brings about a series of unfortunate events. "Krogan Backdraft"
5) Shepard finds his romance is with child. "Rage of the Prego".
6) Shepard's father returns "Family Reunion

When one factors in the new romancing crap,  I think I'm going to have to go with #5 but I'm hoping for #1.  If the character didnt romance anyone then it'll be because one of your crewmates were responsible.
