ME:1 Wrex and Garrus.
ME:2 Kasumi and Zaeed (Tali and Garrus are honorable thirds here) 
ME:3 Garrus and Liara. (Tali is an honorable third) 

Might just be me, but Garrus and Tali got progressively more badass as the series went on. Garrus armed with a Black Widow in ME3 and an insicor sniper in ME2 destroyed -everything-.

Garrus and Liara, although in ME3 I tended to use Javik a lot.

Kind of depends on the class I'm playing - though I will say that Garrus is my most regular squaddie throughout, seemingly regardless of class. Too much of a badass to leave behind.

Since I rarely play soldiers and often play biotics, probably Wrex (ME1), Zaeed (ME2) and James (ME3) take up the other slot most often, though there is a fair bit of rotation.

I don't have characters which I take along most. However, Zaeed and Grunt mostly stayed on board. Not interested in their characters at all. I noticed that I take Javik with me a lot, because I am interested in the background info of the previous cycles.

Sooo, yeah. It is kinda strange that some people like Thane are so low, but oh well. This is mostly combat performance. Garrus dominates. Liara, Miranda, Javik and Wrex doing good. Kasumi and Zaeed being so high is commendable for DLC characters especially since Kasumi is a lightly armored thief and Zaeed is an uber soldier. Predicatbly AShley and Jacob are pretty much hated by all. Morinth is also not surprising,

ME3: Garrus, ...I can't remember. As long as my sniper buddy was there, I was always fine....that and the games weren't that hard. Come to think of it, ME1 was the only hard one.

I did two playthroughs of ME1, one playing with Wrex as a squadmate, one with Liara and both with Garrus.
I've done five playthroughs of ME2, and used Miranda on all five, Garrus on three, and Samara on two.
I've done two full playthroughs and one partial playthrough of ME3, and used Garrus and Liara on all of them.

Yeah, as you can see, once I pick my two mates, I stick with them through the entire game.

ME2: Miranda (squad power) & Grunt ( Brute force )
ME3: Garrus (BFF/Sniper) & Javik ( biotic combos)

In ME 1 and 2 I tended to switch the 2nd squadmate quite a bit.
