 I'll list and update when i find them. feel free to add your own. I'm doing the first few from memory so i don't know  the names of all of them or remember what they were, but I know where they were.

1st level
Pistol power unit: In shop where you meet merc nemesis after dropping down laddet in to market.
Forget what: shop right before overiding door.
Forget the name: in shop to right of area after bypassing door.
Assault rifl ULM: after meeting up with romance option in car lot, turn around and look down.

Casino
Not an item but after getting the camera hacking software you can hack the atms in the casino for 3000 credits and some renegade points.

Archive
Shotgun ULM: first room on table near medi-gel station
SMG power mod: before catwalks after blue scanners on desk in the middle of the room.
Lancer: in vault when you ride the platform, hard to miss.
Sniper high velocity barrel: back of room where glyph talks abpout 18 historical events, opposite doors.
sniper ULM: room where merc's yell Shepard's still alive, same area as the genophage hologram.
AR ULM: same area as sniper ULM, between holograms.

landing pad
SR ULM: top of second ladder near medi-gel station.
SG ULM: after second ladder go right.

Normandy
Merc payment: 15000 credits, galaxy map console near elevator
AR thermal scope: scanning station next to elevator
Campbell's favorite cup: other side of scanner from thermal scope.

Does anyone know if you can gain any mods from playing any arcade or casino games. I've done the main mission and the combat arena objectives and I'm still missing several mods.

You can buy a lot of mods at the normal shops, they get added after the mission but they seem to cap out at IV. I think the rest have to be maxed at the stupid arcade prize game. :(

Yeah I already bought the one's in the shops. I really hope you don't have to play the arcade games to get the rest. Those games suck and I suck at them.

If you get 9999 points in the Arena, you get one of three full armour suits, the Cerberus Nightmare, Cerb Shade and Cerb Spirit, you can only pick one a playthrough though.

The crane game will give you mods if you get an orange prize.

I think the rest of the arcade games give out a mod if you get the high score.

I don't recall if doing the Arena challenges gave me mods or not.

Ha yeah.  I managed to get one mod, one Volus plush toy, and about 10 songs for my apartment.

Actually Shattered Eezo was what I blew the most money on.  Aria hax.

The OP is not 100% accurate. Here is the complete list of mods/collectables and their locations. (Some help from the ME Wiki) This is for the story line only. I have no idea what the mods are from the orange items in the crane machine. Also, you get an armor for getting max points in the Arena. You can only choose one but can get another on a new game+, and then the third on another ng+.

Citadel Wards

-M-11 Suppressor: Received automatically at the start of the mission.

Pistol Power Magnifier: After falling into the lower wards, after the second firefight, in a store called Ryuusei.

-Assault Rifle Ultralight Materials: After using Spectre status to bypass the door, the mod will be on display in a store called Cepheid Variables.

-Sniper Rifle Ultralight Materials: After meeting Shepard's love interest (if squadmate) or Liara (if unromanced or love interest is not a member of Shepard's squad), on the floor opposite the used skycars.

Citadel Archives

-Shotgun Ultralight Materials: In the first area, on a desk near a medical station.

-SMG Power Magnifier: After passing through the security scanners, on desk near a med kit.

- M-7 Lancer: On the tram into the vault.

- Sniper Rifle High-Velocity Barrel: Move past the holo archive about the Battle of the Citadel to the back of the next room. The mod is on a desk against the far wall.

- Assault Rifle Ultralight Materials: In the corridor connecting the holo archives about the genophage with that of the first Spectre.

- Sniper Rifle Ultralight Materials: From the previous mod, proceed towards the door out of the area. The mod is to the left along the way.

Citadel Docks/Normandy

-Sniper Rifle Ultralight Materials: When the Normandy starts getting ready to take off, before going down the ladder, near a med station.

- Shotgun Ultralight Materials: After climbing down the ladder from the previous mod, straight ahead on some crates.

- Assault Rifle Thermal Scope: Aboard the Normandy, at the security checkpoint that leads to the War Room.

- Private Campbell’s Favorite Cup – Security room in between the CIC and the War Room on the Normandy. On the table.

I am stumped on some of these mods.

Like the Sniper Rifle High Velocity Barrel. You get one from the missions, you can buy 2, and then... I won all 4 mods in the claw game and have the high score for Shattered Eezo and Relay Defense, but I don't think any of those were HVB.

 I have play the arcade games a lot and I got weapon mods from mostly the claw machine I haven't tried the mech battle though.
