For the DLC areas, they show up as yellow icons on the map, instead of the white/beige color the rest are.

There is a travel location in the mountains, called Sulcher's Pass. It should also be listed in your Quests under "Premium Content."
If you don't see any of this after leaving Lothering, watching a brief dream video and waking up in your party camp, try disabling and re-enabling the DLC, restarting the game, clearing your download cache (in the Configuration option from the Launcher in the PC version), loading from an old savegame (rather than using Resume), or any combination thereof. I did all of those things together, and it fixed my problem.
